Job summary

HealthHero is recruiting independent Prescribing Clinicians to support an Integrated Urgent Care service across Shropshire, Telford and Wrekin. Roles include face-to-face, home visit, and regional triage shifts, with remote senior clinician support available.

You will assess, diagnose, treat, and refer patients in out-of-hours settings, while working with a multidisciplinary team to improve patient flow and safety in urgent care pathways.

Qualifications

  • Autonomous practitioner with strong clinical assessment and decision-making skills.
  • Experience with adult and paediatric patients, including remote consultations.
  • Minimum 2 years prescribing experience with understanding of urgent care pathways.
  • MSc in Advanced Practice or equivalent experience; non-medical prescribing qualification.

Responsibilities

  • Provide autonomous clinical assessment, diagnosis where possible, and treatment.
  • Accurately triage and prioritise patients using a range of assessment techniques.
  • Undertake home visits with a fully equipped medical car and driver.
  • Refer patients to appropriate care settings and support Care Coordination.
  • Assist triage of NHS111 calls during core hours.

Further to recent organisational growth, we have an exciting opportunity for independent, autonomous Prescribing Clinicians to support our Urgent Care service within the out-of-hours (OOH) period across the Shropshire, Telford and Wrekin locality, Our Clinical Practitioners provide care through the assessment, diagnosis, treatment and discharge or referral of patients with undifferentiated and undiagnosed injury/illness, or acute exacerbation of existing conditions. You will assist in the development of an integrated approach to the delivery of patient care across the whole system of health and social care, working in partnership with all relevant stakeholders, Further to recent organisational growth, we have an exciting opportunity for independent, autonomous Prescribing Clinicians to support our Urgent Care service within the out-of-hours (OOH) period across the Shropshire, Telford and Wrekin locality. Part and full time rota patterns available Working from our Telford Headquarters and/or from one of our Primary Care Centres, as an Independent, autonomous Prescribing Clinician you will work as part of our multidisciplinary team to deliver safe and timely care to those accessing our Integrated Urgent Care Service, providing a variety of shift opportunities to include:

  • Face-to-face shifts based at one of our Primary Care Centres across the Shropshire, Telford and Wrekin locality.
  • Mobile shifts undertaking home visits to patients who are unable to travel to a primary care centre, supported with a fully equipped medical car.
  • Triage shifts based at our regional Head Office.

Our service works to coordinate health and care services to patients and where clinically appropriate, utilise alternative pathways to emergency admission.

About the role Our Clinical Practitioners provide care through the assessment, diagnosis, treatment and discharge or referral of patients with undifferentiated and undiagnosed injury/illness, or acute exacerbation of existing conditions. You will assist in the development of an integrated approach to the delivery of patient care across the whole system of health and social care, working in partnership with all relevant stakeholders. The key elements of the role include but are not limited to:

  • Working as an autonomous practitioner providing clinical assessment, diagnosis (where possible), and treatment (where appropriate) for adult and paediatric patients invited to attend our Primary Care Centres or during remote consultations.
  • Accurately triaging & prioritising all patients, demonstrating the use of a variety of techniques to elicit the history of an event / illness including past medical and drug history.
  • Undertaking home visits to patients who are unable to travel to a primary care centre, supported with a fully equipped medical car and a driver.
  • Referring patients to alternative care settings, where appropriate.
  • Supporting our Care Coordination Service from our regional Head Office.
  • Supporting with triage of calls that have come via the NHS111 pathway during the core daytime surgery opening hours.

Please note that the support of a senior clinician is always available remotely during your shift.

About you

You will be a registered nurse, paramedic, or other allied health professional, with a non-medical prescribing qualification in the context of a broad scope of practice.

  • Ability to work autonomously both in-hours and out of hours.
  • Achieved or working towards an MSc in Advanced Practice.
  • Evidence of an equivalent accumulation of relevant experience and knowledge.
  • Training and experience of physical assessment and clinical reasoning with patients of all ages (including paediatrics).
  • Ability to demonstrate a wide knowledge base and competency in clinical practice.
  • Understanding of the emergency care pathways.
  • Knowledge of primary health care issues affecting a diverse local population.
  • Will have at least 2 years worth of prescribing experience.
  • Completion of a clinical reasoning module e.g. PADRIP or equivalent, Experience working within urgent care, primary care or emergency care pathways
  • Strong clinical reasoning and triage skills, with the ability to manage risk and uncertainty
  • Ability to work autonomously and make complex decisions under pressure
  • Understanding of system pressures and patient flow within urgent and emergency care
  • Experience supporting or working alongside ambulance services or care coordination functions (desirable)
  • MSc in Advanced Practice (achieved or working towards), or equivalent experience
  • Minimum of 2 years prescribing experience
  • Completion of a recognised clinical reasoning module (e.g. PADRIP or equivalent)
