Job summary

BrisDoc Healthcare Services in Bristol is seeking a Clinical Practitioner to deliver autonomous prescribing and high-quality primary medical care across Severnside Integrated Urgent Care, including The Burren, CAS and CMDU. You will work within an MDT, manage day-to-day needs of patients discharged from secondary care, and operate within your professional scope of practice.

Job description

The Clinical Practitioner will work across Severnside Integrated Urgent Care (IUC), including The Burren, Out of Hours, Clinical Assessment Service (CAS), Clinical Medication Delivery Unit (CMDU) and Frailty Assessment and Coordination for Emergency and Urgent Care (ACE).

The Burren is an innovative collaborative service bringing together secondary, community, urgent and primary care within an intermediate care setting. The post holder will be an autonomous prescribing clinician, providing high-quality primary medical care while working within their professional scope of practice.The role involves managing the day-to-day clinical needs of patients discharged from secondary care into community placements with rehabilitation needs. The successful candidate will have advanced clinical assessment and decision-making skills, be confident in managing clinical risk, and work independently while contributing to excellent patient outcomes. Experience in urgent care and chronic disease management would be advantageous.

Supported by the Nursing and AHP Leadership Team and Primary Care GPs, the Clinical Practitioner will provide patient-centred care and ensure individual health needs are met.

As a key member of the multidisciplinary team (MDT), the post holder will participate in ward rounds and work closely with Nurses, GPs, Therapists, Social Care and Operational colleagues to support patients and deliver the right care at the right time.

Main duties of the job

• ClinicaloAssess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis, complex needs, acute and chronic conditions for both physical, mental health and psycho-social needs.
• Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
• To undertake visits to patient's homes as requiredCompose and ensure robust safety netting and risk management for patients presenting to the service.
• Make effective referrals to system partners and other care providers to ensure a holistic provision of care for patient's needs.
• Participate in team discussions to ensure and drive best practice, using the best available evidence to provide exemplary world class care to every patient.

Prescribers Only

• Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice

CMDU Only

• This is a service for North Somerset prescribing antivirals for Covid. This service is run during set hours and has one allocated clinician.

Communication

• Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment
