Job summary
We have an exciting opportunity available for a Clinical Pharmacist to join our small but ever-growing GP Practice in Yate. As an integral part of a multi-disciplinary team, and with support of a GP Partner clinical mentor, in this varied role you will be responsible for providing structured medication reviews for patients, supporting long-term condition management, as well as project work such as CQC compliance, community collaboration and the overall management of prescribing systems.
This is a part time role, working 17 hours per week consisting of a mix of remote working and patient facing work in the practice.

We are a progressive, forward thinking, innovative and growing Practice, based in South Gloucestershire. Our practice is in the town of Yate, which lies equidistant (12 miles) between the vibrant, cultural, and historic cities of Bristol and Bath. We have developed a fantastic reputation within the local community and strive to provide excellent care.
With a patient list size of approximately 6000, the clinical team comprises of 3 salaried GPs, 2 GP Partners, a Physician's Associate, Paramedics, Mental Health Nurse, Physiotherapist, Social Prescriber and practice nurses.
