Job description

Clinical Pharmacist - Wanstead and Woodford PCN

We recognize the importance of Clinical Pharmacists in Primary Care and the key role they play in supporting the delivery of the Network Contract DES and Quality Outcomes Framework (QOF). They deliver a range of health interventions at Practice level, including developing and managing medicine optimisation services within Primary Care, systems for safer prescribing, structured medication reviews (e.g., for patients with polypharmacy), reviews of patients with long-term conditions within their competency, and withdrawing medicines no longer needed through NHS-led programmes. For Enhanced Health in Care Homes, residents will benefit from regular clinical-pharmacy-led medicines reviews.

The Clinical Pharmacist plays a crucial role in our general practices and is of utmost importance to our local patient population. It serves as a cornerstone in our pursuit of enhancing health outcomes.

Main duties of the job

The post holder is a pharmacist, supporting and working alongside a team of pharmacists in general practice. They will be supported by a senior clinical pharmacist who will develop, manage, and mentor them, and by a GP who will provide clinical supervision and act as line manager.

To provide clinical leadership on medicines optimisation and quality improvement, as well as managing medicines safety and antimicrobial stewardship.

To work as part of a multidisciplinary team in a patient-facing role, taking responsibility for areas of chronic disease management within the practice, and undertaking clinical medication reviews to proactively manage patients with complex polypharmacy.

Job responsibilities

The post holder will support general practice staff with prescription and medication queries, assist with the repeat prescription system, medicines reconciliation on transfer of care, and systems for safer prescribing. They will provide expertise in clinical medicines advice, addressing public and social care needs of patients in the GP practice(s).

The post holder will ensure the practice integrates with community and hospital pharmacies to utilize skill mix, improve patient outcomes, ensure better access to healthcare, and manage workload. The role involves organizing practice-based meetings with local community pharmacists to improve collaboration and support patients in accessing and optimally using their medicines.

Qualifications
  • Masters degree in pharmacy (MPharm) or equivalent
  • Registration with the General Pharmaceutical Council
  • Post-graduate clinical diploma level or equivalent training/experience or independent prescriber
  • A member of or working towards Faculty membership of the Royal Pharmaceutical Society
