Clinical Pharmacist – Salisbury
A well-established and forward-thinking healthcare provider in Salisbury is currently seeking an experienced and motivated Clinical Pharmacist to join their dynamic team. Position Overview:
The successful candidate will work closely with GPs, nurses, and other healthcare professionals to ensure safe, effective, and efficient use of medicines. They will play a pivotal role in managing long-term conditions, conducting medication reviews, supporting prescribing processes, and improving overall patient outcomes.
Key Responsibilities:
Conduct clinical medication reviews and support patients in managing their medication regimes effectively.
Provide expert advice on prescribing issues, particularly in the management of long-term conditions.
Collaborate with the multidisciplinary team to ensure safe and effective prescribing across the practice.
Assist in the development of protocols and guidelines for medicine management.
Contribute to audits, quality improvement initiatives, and education of clinical staff.
Monitor and manage repeat prescribing systems and promote cost-effective prescribing.
Candidate Profile:
Registered with the General Pharmaceutical Council (GPhC).
Demonstrated experience in a clinical setting, ideally within primary care.
Independent prescribing qualification (or working towards) is desirable.
Excellent communication and organisational skills.
A commitment to patient-centred care and professional development.
