3. Duration: Ongoing Contract
4. Schedule: Monday to Friday, 9:00 AM - 5:30 PM
5. Start Date: June 2025
6. Rate of Pay: £30 - £35 per hour

What You'll Be Doing:

Join our team as a Clinical Pharmacist, supporting a GP surgery and making a direct impact on patient care and medicines optimisation. Your responsibilities will include:

1. Conducting medication reviews to improve patient treatment outcomes
2. Providing prescribing support and guidance to GPs
3. Managing repeat prescriptions, ensuring safety and accuracy
4. Overseeing medicines reconciliation for new and discharged patients
5. Supporting long-term condition management (e.g., diabetes, hypertension)
6. Answering prescription queries and collaborating with healthcare teams
7. Working onsite to deliver high-quality pharmacy support

Requirements & Qualifications:

To be successful in this role, you will need:

1. Prior GP experience (essential)
2. Independent Prescribing qualification (preferable)
3. Strong expertise in medicines optimisation and patient consultations
4. Proficiency in GP pharmacy systems (EMIS & SystmOne)
5. Excellent communication and teamwork skills
