Job summary

NHS Jobs is seeking a pharmacist to undertake acute and non-urgent clinical consultations both face-to-face and by telephone during contracted hours. The role involves same-day triage, processing EPS requests, reviewing hospital letters, and acting on pathology results.

You will conduct comprehensive medication reviews and support audits, with responsibility for cost-effective prescribing. You'll review prescribing data regularly, ensure adherence to local guidelines, and plan seasonal flu

Responsibilities

• Undertake acute and non-urgent clinical consultations (face-to-face and via telephone).
• Carry out same-day acute triage using the clinical system during contracted hours (08:00 to 18:30).
• Process Electronic Prescription Service (EPS) requests accurately and efficiently.
• Review and action Docman tasks, including hospital letters and other clinical correspondence.
• Review pathology and blood test results, taking appropriate clinical action.
• Conduct structured medication reviews: face-to-face; via telephone; for housebound patients; for patients in nursing and residential care homes.
• Regularly review prescribing data to ensure clinical appropriateness and cost-effectiveness.
• Prescribe in accordance with local prescribing guidelines. Support clinical audits related to prescribing and medicines management.
• Plan and deliver seasonal flu vaccination clinics.
