Summary:

As a Clinical Pharmacist at NHS Jobs, you will play a crucial role in the practice-based team, focusing on medication reviews, prescribing improvement, and patient care in high-risk groups. Your responsibilities will include clinical assessment, treatment, and proactive management of patients with chronic diseases, especially in vulnerable populations. Qualifications & Training:

• Completion of an undergraduate degree in Pharmacy and registration with the General Pharmaceutical Council.
• Minimum of 5 years post-graduate experience in pharmacy.
• Member of the Royal Pharmaceutical Society (RPS) and working towards RPS Faculty membership.
• Hold an independent prescribing qualification and a postgraduate pharmacy qualification.
• Qualified from an accredited training pathway for clinical pharmacists in primary care settings.
