Summary: This is a developmental role with a grade commensurate with experience, knowledge, skills, and competencies acquired. The position involves providing support for the clinical work of senior clinical pharmacists across various services, including Chemotherapy/Haematology, Mental Health, Urgent Care, Medicines Advice, Patient Services, and rotating responsibilities across all wards. Competencies required for this support will be gained through the pharmacist rotational training programme.

The role encompasses Medicines Reconciliation and drug history transcribing to Trust standards. Ensuring systems for facilitating discharge medicines meet patient and general practitioner needs is a key responsibility. Effective communication and collaboration with pharmacy staff, nursing staff, Allied Health Professionals, Medical staff, and GPs are essential to ensure clear and timely information exchange.

Additionally, the position involves clinical management, support, and supervision for pre-registration pharmacists, clinical placements, and undergraduate vacation pharmacy students. Professional development opportunities may include palliative care and pediatrics. The Lead Clinical Pharmacist will conduct a yearly performance appraisal to review the job description, assess development and training needs, evaluate performance over the year, and establish objectives for the following year.
