Summary

We are seeking a dedicated Clinical Pharmacist to join our dynamic team at NHS Jobs. This is a developmental role tailored to your experience, knowledge, skills, and competencies. The successful candidate will provide essential support to senior clinical pharmacists across a variety of services.

Key Responsibilities

Support Services: Work with Chemotherapy/Haematology Services, Mental Health Services, Urgent Care Services, Medicines Advice services, Patient Services, and various wards on a rotational basis.

Medicines Reconciliation: Conduct medicines reconciliation and drug history transcribing in accordance with Trust standards.

Discharge Medicines Facilitation: Ensure the efficient management of discharge medicines, meeting the needs of patients and general practitioners.

Liaison: Maintain clear and timely communication with pharmacy staff, nursing staff, Allied Health Professionals, medical staff, and GPs.

Clinical Management: Provide support and supervision to pre-registration pharmacists, clinical placements, and undergraduate vacation pharmacy students.

Professional Development: Engage in professional development opportunities, potentially including palliative care and paediatrics.

Performance Appraisal

The Lead Clinical Pharmacist will conduct an annual performance appraisal. This will be an opportunity to review your job description, development and training needs, performance over the past year, and to agree on objectives for the following year.
