Summary

As a Clinical Pharmacist at NHS Jobs, you will be responsible for conducting medication reviews for patients with single or multiple long-term conditions, focusing on medicines optimization. This role involves reconciling medicines post-discharge, collaborating with patients and community pharmacists, and contributing to multidisciplinary clinical meetings within the PCN. You will participate in out-of-hours services, manage repeat prescriptions, and ensure appropriate monitoring tests are in place.

Responsibilities
  • Conduct clinical medication reviews for patients with long-term conditions
  • Reconcile medicines post-discharge to reduce the risk of readmission
  • Participate in multidisciplinary clinical meetings within the PCN
  • Provide out-of-hours/on-call/extended services, including patient consultations
  • Manage repeat prescription requests and resolve queries within scope of practice
  • Implement changes to medicines in line with guidelines and alerts
  • Maintain complete records of patient contacts using clinical templates
  • Take a proactive role in audit and quality improvement
  • Provide independent information to patients regarding medicines
  • Participate in policy reviews related to medicines management and prescribing
  • Identify clinical risks at the network level and make recommendations for improvement
  • Monitor network practices prescribing against local health economy standards
  • Maintain continuous professional development and competence
