Key Duties
- Provide remote services to patients registered with the Hub.
- Provide information and support to patients on:
- Medicines Information - General advice requests
- Pharmaceutical advice
- Repeat Prescriptions
Support primary care sites and assist with prescribing and high-risk drug management audits as required.
Use critical thinking and effective communication skills to assess callers' needs, considering social, cultural, and economic factors.
Prescribe appropriately for urgent prescriptions, using access to summary care record information for accurate re-prescribing.
Communicate information and pharmaceutical advice clearly and understandably, tailored to individual needs.
Identify and manage clinical risks related to health and medicines queries, in line with professional responsibilities.
Identify and act on child and adult protection issues arising from calls, following protocols.
Record accurate and timely documentation of advice given and complete electronic records using the appropriate clinical system.
Access specialist information through electronic databases (e.g., BNF) to ensure accurate advice.
Communicate effectively with colleagues to support service delivery.

Key Skills, Qualifications, and Client Requirements
- Registered with the General Pharmaceutical Council (GPhC)
- Non-Medical Prescriber
- Successful completion of CPPE General Practice Pharmacist pathway
- Commitment to ongoing professional development
- Extensive experience in primary care
- Ability to provide both face-to-face and remote patient care
- Effective workload management and support for pharmacy tasks
