Job Summary

Join the EAST:MK Primary Care Network as a full-time clinical pharmacist, collaborating with our multidisciplinary team across four surgeries. Your role involves ensuring safe, evidence-based, and cost-effective prescribing, alongside offering support on medication queries and conducting medicines reconciliation.

Main Duties of the Job

Work within a patient-facing role in a multidisciplinary team

Manage areas of chronic disease management proactively

Support delivery of the Network Contract Directed Enhanced Service

Conduct structured medication reviews for patients with complex polypharmacy

Provide clinical medicines expertise to patients and practice teams

Improve prescribing practices and service quality through clinical audits

Interface with community and hospital colleagues to resolve medication-related issues

Assist in delivering enhanced health-in-care-homes initiatives
