About The Role

Ortus Health is seeking a skilled Clinical Pharmacist - Medicines Reconciliation to join our dynamic team in a fully remote capacity. In this role, you will play a vital part in ensuring medication safety and continuity of care by completing medicines reconciliations from clinical correspondence, including hospital discharge summaries and outpatient clinic letters.

Working as part of our remote multidisciplinary team, you will review incoming clinical documents to identify and reconcile changes to patients' medication regimens. This includes assessing for discrepancies, potential drug interactions, and ensuring that any updates are accurately actioned within the patient record.

Key Responsibilities
  • Complete thorough medicines reconciliations using clinical correspondence, including hospital discharge summaries and outpatient letters
  • Identify and resolve discrepancies between current medication records and updated hospital communications
  • Review and document medication changes, initiating appropriate actions or flagging for GP follow-up where required
  • Collaborate remotely with GPs, care coordinators, and practice teams to support safe and accurate prescribing
  • Use clinical systems and decision-support tools to check for potential drug interactions, duplications, or contraindications
  • Maintain accurate clinical documentation in line with practice protocols and professional standards
  • Engage in ongoing professional development to ensure up-to-date knowledge of medicines optimisation and primary care best practice
Qualifications
  • Must be a registered pharmacist with GPhC registration
  • Must be an independent prescriber
  • Must have completed CPPE pathway
  • Excellent analytical and problem-solving skills with attention to detail
  • Effective verbal and written communication abilities for patient and team interactions
  • Demonstrated ability to work independently and manage multiple tasks remotely
