Job summary

Collingham Church View Surgery is seeking a Prescribing Clinical Pharmacist to lead medicines optimisation, carry out structured medication reviews and reduce risk of harm for patients.

Based in a well-established GP practice, you will collaborate with GPs, nurses and admin staff to review long-term conditions, respond to MHRA alerts and support safety with a focus on quality improvement and patient-centred care. Independent prescriber status and primary care pharmacy experience are essential.

Qualifications

Independent Prescriber qualification recognised by the GPhC.
Membership of Primary Care Pharmacy Association (PCPA).
Membership of the Royal Pharmaceutical Society.

Responsibilities

Support the practice with medicines optimisation.
Carry out patient-facing or telephone medication reviews.
Structure medication reviews with direct patient contact.
Management of long-term conditions.
Reviewing systems for safer prescribing.
Contributing to repeat prescriptions authorisation & reauthorisation.
Actioning acute prescription requests.
Contributing to achievement of QOF and local quality improvement schemes.
Undertaking clinical audits.
