A leading healthcare provider seeks a Clinical Pharmacist to enhance medicines optimisation services across Grantham practices. This role involves collaboration with a multidisciplinary team to manage prescriptions and improve patient care quality. The successful candidate will receive support for professional development and mentoring in a patient-facing environment.
As a Clinical Pharmacist, you will work with our current Clinical Pharmacist Team to further develop and manage medicines optimisation services within our practice. You will work in a small number of the Grantham practices as well as working from home on occasion. You will therefore have the opportunity to build relationships with the primary care team. You will collaborate to manage repeat prescription authorisation and reauthorisation, acute prescription requests, management of transfer of care, and systems for safer prescribing. You will have one day a week dedicated to project work, collaborating with the four Pharmacy Technicians across the whole PCN with support from our three senior CPs.
You will have the opportunity to develop areas of clinical interest in a patient-facing role, enhance your prescribing skills, and improve the management of patients with long-term conditions. Mentoring and support will be provided.
You will be supported through your CPPE pathway and your IP course if needed by our team of 12 Pharmacists. You will not work in any practice alone.
You will provide primary support to general practice staff regarding prescription and medication queries. You will support the repeat prescription system, handle acute prescription requests, conduct medicines reconciliation on transfer of care, and contribute to systems for safer prescribing. You will provide expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practice(s).
You will provide clinical leadership on medicines optimisation and quality improvement initiatives, and manage aspects of the quality and outcomes framework and enhanced services.
You will ensure the practice integrates with community and hospital pharmacy to optimize skill mix, improve patient outcomes, ensure better healthcare access, and help manage workload. The role is pivotal to improving care quality and operational efficiency, requiring motivation and passion to deliver excellent service within general practice.
You will collaborate with other CPs in our large team, receiving full support throughout.
We are a GP Federation covering 16 practices—10 in Grantham and surrounding villages, and 6 in Sleaford. We serve over 130,000 patients across all sites. Our team includes 12 CPs, including 3 senior CPs and 6 pharmacy technicians. Each practice has at least one CP to support and mentor staff as needed.
Our CP team members are at various stages of completing their CPPE/IP courses and are happy to share their experiences with K2 Healthcare.
The post holder is a pharmacist operating within professional boundaries, supporting and working alongside the practice's Pharmacist and Dispensary team to develop, manage, and mentor them.
The post holder will work as part of a multidisciplinary team in a patient-facing role, taking responsibility for chronic disease management within the practice and conducting clinical medication reviews to proactively manage patients with complex polypharmacy.
The post holder will support general practice staff with prescription and medication queries, assist with the repeat prescription system, handle acute prescription requests, conduct medicines reconciliation on transfer of care, and contribute to systems for safer prescribing, providing clinical medicines advice addressing public and social care needs.
The post holder will provide clinical leadership on medicines optimisation and quality improvement, managing aspects of the QOF and enhanced services.
The role involves ensuring integration with community and hospital pharmacies to utilize skill mix, improve patient outcomes, enhance healthcare access, and manage workload. This role is central to improving care quality and operational efficiency, requiring motivation and passion for delivering excellent service.
The post holder will be supported in developing towards becoming a non-medical prescriber if desired.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975. A disclosure check will be required to screen for any previous criminal convictions.
Salary: £45,000 to £46,000 per year (£45K starting, rising to £46K upon IP and CPPE completion).
