Job Overview
The CRT Pharmacy team was set up in 2019 providing services to Newport, Caerphilly and Torfaen. Due to the continued success of the service, further investment was secured to extend the service to all 5 boroughs, and three Community Hospital rehabilitation wards in 2023. This ambitious expansion involved recruitment of a team now consisting of 16 posts leading to a balanced skill-mix of pharmacy staff.
A vacancy has arisen for a Clinical Pharmacist, and due to the flexibility of current staff, this vacancy can be based in Newport, Blaenau Gwent or Chepstow Community Hospital depending on the successful candidate's preference.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
The team currently provide a Pharmacy service to Frailty Consultant-led ‘hospital at home’ patients, Therapies/Social Care-led Reablement patients, Nurse-led home parenteral service patients (including IV antimicrobials) and Community Falls patients.
The extension of the service in 2023/24 also led to the exciting integration and redesign of the Pharmacy service provided to the Community Hospital rehabilitation wards of South Monmouthshire (Chepstow) and Torfaen.
We also take referrals from acute ward Pharmacy staff to ensure that frail/vulnerable patients are supported in taking medicines post discharge. Our role involves being fully immersed in the MDT with regular meetings, home visits, consultant led virtual ward rounds and actual ward rounds, and data collection/analysis of contributions to patient care. Each day brings a new challenge, the chance to learn from those around us, job satisfaction and an opportunity to further develop the service!
With a unique role working across the primary/secondary care interface we have built excellent relationships with community, practice based and hospital colleagues. Our team also contributes to National and UK groups working in the field of Frailty/Virtual Wards/Community Resource.
