An exciting opportunity exists to join our clinical pharmacy team at the Borders General Hospital, providing pharmaceutical care to our frail elderly population throughout NHS Borders. This will involve working with the multidisciplinary team providing comprehensive geriatric assessment with our Frailty services at the Front Door, inpatients on the Care of the Elderly wards, and through referral pathways our frail patients within our other specialities, including Cancer care, Palliative care, Mental Health, Medicine and Surgery.

You will also help develop and review referral pathways for frail patients on discharge for ongoing pharmacy input by the wider team across NHS Borders within Hospital at Home, Care Homes, and our Integrated Care and Primary Care teams.

As part of our clinical team, you will work closely with multidisciplinary staff and also contribute to the developing role of our clinical technicians and pharmacy support staff. You will promote robust medicines governance, patient safety initiatives and future developments such as electronic medicines management.

You should have relevant clinical experience and have completed Post Registration Foundation Training or demonstrate evidence of clinical knowledge equivalent to that level. You will be an independent prescriber or working towards this qualification. You will be encouraged to undertake continuous professional development or further postgraduate study depending on your level of experience and needs.
