Job summary

Woodlands Hospital in Darlington is seeking a Clinical Pharmacist to join the Pharmacy Department on a 22.5-hour per week permanent basis. The role involves conducting medication reviews, advising patients on treatment plans and supporting safe prescribing within guidelines.

The successful candidate will have an MPharm (or equivalent), GPhC registration and experience in a clinical hospital pharmacy. EPMA proficiency is preferred, and the post offers competitive benefits within Circle Health

Qualifications

  • MPharm or BSc/BPharm degree required.
  • GPhC registered as a Pharmacist.
  • Previous experience in a clinical hospital pharmacy role.
  • Strong knowledge of therapeutics, guidelines and pharmaceutical legislation.
  • Excellent communication and clinical decision-making skills.
  • Ability to work autonomously and in a multidisciplinary team.
  • Confident using electronic prescribing and medication administration systems (EPMA).

Responsibilities

  • Conduct medication reviews for inpatients and outpatients, identifying interactions and opportunities for medicines optimisation.
  • Provide tailored advice to patients on treatment plans, side effects and lifestyle considerations.
  • Support safe prescribing through reconciliation, clinical screening and dose adjustment.
  • Review and authorise prescriptions in line with clinical guidelines, formulary restrictions and regulatory requirements.
  • Promote safe and cost-effective use of medicines by adhering to policies.
  • Collaborate with procurement, stock control and pharmacy technicians to ensure medicine availability.
  • Monitor and report medication incidents, near misses and ADRs per MHRA guidance.

Salary: Up to £48,300.00 per year (depending on level of experience, training and qualification)

We have an opportunity for a Clinical Pharmacist to join their team of staff in the Pharmacy Department.

This is a Part time role for 22.5 hours a week. The role holder will be required to cover a flexible shift pattern.

Duties of this role include:
  • Conduct medication reviews for inpatients and outpatients, identifying interactions, contraindications, and opportunities for medicines optimisation.
  • Provide tailored advice to patients on their treatment plans, including side effects, compliance, and lifestyle considerations.
  • Support safe prescribing through reconciliation, clinical screening, and dose adjustment.
  • Review and authorise prescriptions in line with clinical guidelines, formulary restrictions, and regulatory requirements.
  • Promote safe and cost-effective use of medicines by supporting adherence to local and national policies.
  • Collaborate with procurement, stock control, and pharmacy technicians to ensure medicine availability and supply continuity
  • Monitor and report medication incidents, near misses, and adverse drug reactions in line with hospital protocols and MHRA guidance
Applicants should meet the following criteria:
  • A degree in pharmacy MPharm, BSc or BPharm.
  • GPhC registration as a Pharmacist.
  • Previous experience in a clinical hospital pharmacy role.
  • Strong knowledge of therapeutics, clinical guidelines, and pharmaceutical legislation.
  • Excellent communication and clinical decision‑making skills.
  • Ability to work autonomously and as part of a multidisciplinary team.
  • Confident using electronic prescribing and medication administration systems (e.g., EPMA).
