Calderdale and Huddersfield NHS Foundation Trust is seeking a Clinical Pharmacist to join the Community Division, delivering specialist pharmaceutical care to patients in their homes or care homes across Calderdale and Huddersfield, supporting Hospital at Home and Urgent Community Response workflows.

You will collaborate with primary and secondary care teams, review medications, reconcile therapy on transfers, and provide patient counselling to optimise concordance and safety.

Competenze

• MPharm or equivalent degree approved by RPSGB.
• Registered Pharmacist with the Royal Pharmaceutical Society.
• Evidence of practical medication review in hospital or community.
• Knowledge of Medicines Management and Pharmacy Practice.
• Ability to prioritise workload and meet deadlines.
• Ability to work under pressure and on own initiative.
• Commitment to CPD.
• Eligible to work in the UK.

Mansioni

• Review medication and work closely with Community Teams.
• Ensure patients derive maximum benefit from prescribed medication.
• Participate in MDT meetings.
• Liaise with GPs and community pharmacists to reconcile medicines after transfer to primary care.
• Communicate recommendations on medication changes to GPs and hospital consultants.
• Provide pharmaceutical counselling to patients in domiciliary care to improve concordance.
• Work independently to support patients with complex medication issues.
• Develop partnership with patient, carers and relatives to reflect patient choice.
• Develop and maintain relationships with healthcare professionals and other agencies.
• Visit patients as a lone worker in their homes or care homes.
• Participate in weekend/holiday/out-of-hours provision of pharmacy services.
