A healthcare organization is seeking flexible Clinical Pharmacists and ANPs for a hybrid role in weight management. Responsibilities include assessing patient eligibility for tirzepatide, managing caseloads, and providing support to patients. Candidates must have GPhC or NMC registration and an Independent Prescriber qualification. This 6-month fixed-term role offers a part-time flexible working pattern.
We are seeking flexible and experienced Clinical Pharmacists and ANPs to support the delivery of a new, targeted NHS weight management service in line with NICE guidance. This small, focused service will initially support around 150 patients meeting strict NHS England eligibility criteria for tirzepatide prescribing in primary care. The role involves reviewing patient suitability and offering support and monitoring whilst titrating. This role is a hybrid clinic/ remote role with all 1st review appointments being offered face-to-face.
Fixed term contract of 6 months with opportunity to extend
Your role will include: Conducting initial in person assessments to confirm eligibility
Managing a caseload of patients through titration
Education and support to patients including administration, potential side effects, and lifestyle modifications.
Escalation of any complex patients, support required for supervising GP(s).

Clinical Pharmacist and ANP - Tirzepatide Weight Management Service
Job PurposeThis role involves assessing patient eligibility for tirzepatide and conducting regular medication reviews to ensure optimal therapeutic outcomes within the Specialist Weight Management Prescribing Service in South and West Hertfordshire in alignment with national guidance
Key Responsibilities
Review patient referrals to assess eligibility and clinical appropriateness for tirzepatide under national cohort one criteria which include body mass index greater than or equal to forty and four or more obesity related conditions
Review clinical information medical history baseline measurements and patient engagement to determine suitability for treatment
Titrate doses appropriately based on clinical response tolerability and monitoring data in line with evidence based practice
Provide information to patients on tirzepatide use including administration potential side effects and lifestyle modifications
Empower patients to engage in long term behaviour change and medication adherence
Conduct regular reviews to monitor effectiveness adherence safety and tolerability of tirzepatide
Adjust treatment plans accordingly and refer to other clinicians as needed
Work collaboratively with the wider team and escalate complex cases to supervising general practitioners
Ensure prescribing is in line with national and local governance protocols and formulary guidance
Report and act on adverse drug reactions or concerns about medication safety through appropriate channels
Maintain clinical records and support compliance with national and local governance requirements
Collaborate with lifestyle support providers to ensure wraparound care is in place and monitored
Attend team meetings and contribute to service evaluation and quality improvement
