Enfield Unity Primary Care Network (PCN) is a large organization representing over 158,000 patients across 17 GP Practice sites. Our goal is to enhance services and outcomes by increasing healthcare access, supporting multidisciplinary teams for our most unwell patients, and championing Clinical Pharmacists in the General Practice sector.

Main duties of the job
This is an exciting opportunity for enthusiastic and forward-thinking local pharmacists to join our established and growing team, providing meaningful, patient-centered support in general practice to deliver:
- Clinical medicine review and long-term condition clinics
- Prescription requests, queries, and prescribing efficiencies
- Safe prescribing practice and audits
The post holder should demonstrate:
- Good IT skills with the ability to analyze complex technical information
- Ability to recognize priorities and identify deviations from normal patterns
- Effective independent and team working skills
- Accountability for delivering professional expertise and direct service provision
About Us
The successful applicant will be based at one or two of Enfield Unity PCN's 17 sites, with a named Clinical Supervisor and Senior Clinical Pharmacist for support. Medicus Health Partners (MHP) is the lead practice and employer, comprising 9 of the 17 PCN practices, providing access to training, support, and governance systems. This role offers a unique opportunity to help shape pharmacy development in general practice, working closely with clinical directors and senior pharmacists.

Job Responsibilities
- Conduct long-term condition clinics, reviewing medicines and monitoring needs
- Undertake clinical medication reviews and provide recommendations
- Support medicines management and safety, including working with care home staff
- Respond to medicine-related inquiries from staff, patients, and healthcare teams
- Review medicines associated with unplanned hospital admissions and manage discharge medicines reconciliation
- Ensure continuity of medicines supply for high-risk patients
- Signpost patients to appropriate care
- Manage repeat prescribing processes and reviews
- Identify high-risk patient cohorts through risk stratification
- Contribute to service development and pharmaceutical advice
- Analyze medicines data to support decision-making
- Undertake audits and implement safety and quality improvements
- Provide education and training on therapeutics and medicines optimization
Person Specification
Qualifications
- Essential: Masters in pharmacy (MPharm) or equivalent
- Desirable: Post-graduate diploma, independent prescriber qualification or working towards it
Experience
- Essential: Minimum 2 years post-qualification experience, understanding of primary care prescribing, and excellent communication skills
- Desirable: Experience in primary care and in-depth therapeutic knowledge

- Essential: Reside locally in Enfield
