Job summary

Twyford Surgery in Berkshire is seeking an experienced clinical pharmacist to join our GP team. You will run independent clinics, prescribe autonomously and help drive safe, effective prescribing for our patient population.

Responsibilities include medication reviews, long-term condition management and collaboration with GPs, pharmacy technicians and the dispensary. Independent prescribing and GPhC registration are essential; EMIS Web experience is desirable. Salary and hours negotiable.

Qualifications

• GPhC-registered pharmacist with Independent Prescribing
• Experience as Independent Prescriber in general practice or primary care
• Able to assess patients, make clinical decisions and run your own clinics
• Experience in medication reviews and managing long-term conditions
• Excellent communication and collaborative working
• Desirable EMIS Web experience
• Experience in prescribing audits and medicines optimisation

• Running independent clinical clinics
• Assessing, managing and prescribing for patients within your scope of competence
• Undertaking structured medication reviews for polypharmacy, frailty and complex needs
• Managing long-term conditions, including hypertension and cardiovascular risk
• Optimising medication and deprescribing where appropriate
• Managing medication queries and high-risk monitoring with pharmacy team
• Reviewing hospital correspondence and implementing changes
• Collaborating with GPs, technicians and dispensary team

Twyford Surgery is a friendly, forward-thinking GP practice serving approximately 11,500 patients. We have an exciting opportunity for an experienced clinical pharmacist to join our friendly supportive team.

About the role

We are seeking an enthusiastic, clinically confident pharmacist who is able to prescribe independently and is comfortable working autonomously, running their own patient-facing clinics and contributing to high-quality, safe and effective prescribing.

Key responsibilities

• Running independent clinical clinics
• Assessing, managing and prescribing for patients within your scope of competence
• Undertaking structured medication reviews, particularly for patients with polypharmacy, frailty and complex medication needs
• Managing long-term conditions, including hypertension and cardiovascular risk
• Optimising medication, identifying opportunities for deprescribing and reducing unnecessary polypharmacy
• Managing medication-related queries, repeat prescribing issues and high-risk medication monitoring alongside our team of pharmacy technicians
• Reviewing hospital correspondence and implementing appropriate medication changes
• Working collaboratively with GPs, pharmacy technicians and the dispensary team
• Supporting prescribing audits, quality improvement and medicines optimisation initiatives

About you

• Essential GPhC-registered pharmacist with an Independent Prescribing qualification
• Experience working as an Independent Prescriber in general practice or a comparable primary care setting
• Confident in independently assessing patients, making clinical decisions and managing your own clinics
• Experience undertaking medication reviews and managing long-term conditions
• Excellent communication skills, clinical judgement and ability to work collaboratively
• Desirable Experience with EMIS Web
• Experience in prescribing audits, quality improvement and medicines optimisation
