A healthcare network in Southend-on-Sea is seeking a Clinical Pharmacist to oversee medicines optimisation and quality improvement. The successful candidate will provide clinical leadership, improve patient outcomes, and collaborate with community and hospital pharmacy. Clinical Pharmacist required for Southend Victoria Primary Care Network. Duties include medicines reconciliation on transfer of care and implementing systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the PCN.
The post holder will provide clinical leadership on medicines optimisation and quality improvement and manage aspects of the quality and outcomes framework and enhanced services.
The post holder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.
Southend Victoria PCN covers nine practices with approximately 60,000 patients in the centre of Southend-On-Sea, Essex. We are a co-operative close knit group of practices who work closely together to deliver services which provide an enhanced level of care for our patients.
Clinical Pharmacist is a new and developing role within General Practice and this is a wonderful opportunity for someone who is passionate about demonstrating how effective this and impactful this will be.
The Queensway Surgery is the lead practice for the PCN. It is a large training practice with the ethos of developing staff to fulfil their potential. There is regular training scheduled for all clinicians to facilitate their development and we insist on good GP supervision for Pharmacists when they are working at any of our GP practices.
You will be joining a team currently consisting of 5 Physician Assistants, 3 Advanced Nurse Practitioners, 3 Care Coordinators, 2 Physiotherapists, 5 GP Assistants, 3 pharmacists, a Dietician, a Paramedic, a Podiatrist all supported by a Clinical Director and dedicated management team.
