A healthcare provider in Sheffield is seeking a qualified pharmacist to manage medicines and enhance patient care across multiple GP practices. The ideal candidate will have a Master’s degree in Pharmacy, strong interpersonal and communication skills, and be registered with the General Pharmaceutical Council. This full-time position offers a competitive salary ranging from £46,800 to £51,000 annually.
The post holder will work independently the majority of the time with support from the Clinical Director and GPs within the Peak Edge Primary Care Network. Engaging with GP Partners and Practice Managers across the 4 Practices covering the Peak Edge Network: Woodseats Medical Centre, Steel City General Practice, Meadowgreen Health Centre, Sloan Medical Centre.

Develop and manage a medicines management plan and deliver patient services as determined by practice/PCN policy and local and national guidance.
Develop and manage the medicines management team, including delivery of training, to maximise cost-effective prescribing and improve the quality of patient care.
Work within clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, conduct face-to-face structured medication reviews, manage long-term conditions, manage medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations and reauthorisation, process acute prescription requests, and address both public health and social care needs of patients in the GP practices that make up the PCN.
Undertake medication reviews for patients with polypharmacy, especially older people, those in residential care homes, and individuals with multiple co-morbidities.
Provide leadership on quality improvement and clinical audit, and manage aspects of the Quality and Outcomes Framework (QOF).
Provide medication review services to patients via clinics in the practice, domiciliary visits, and in residential and nursing homes; deliver pharmaceutical care plans that maximise cost-effective prescribing and improve patient care.
Present at patient group meetings or other events to advise on appropriate use of medicines.
Produce patient information leaflets and posters and run medicine awareness projects throughout the year.
Assist partners with monitoring and managing prescribing budgets.
Prepare evidence-based resources to support the medicine management team and other health professionals in implementing rational cost-effective prescribing.
Help plan, develop, and support the introduction of new working processes within the practice to optimise prescribing quality.
See patients with single or multiple medical problems where medicine optimisation is required (e.g., Respiratory, Cardiovascular, Diabetes).
Review ongoing need for each medicine, monitoring needs, and support patients in taking medicines to maximise their use (medicines optimisation).
Make appropriate recommendations to Senior Pharmacists or GPs for medicine improvement.
Undertake clinical medication reviews with patients and provide recommendations for senior clinical pharmacists, nurses, and/or GPs on prescribing and monitoring.
Work with care home staff to improve safety of medicines ordering and administration.
