A regional healthcare network in England seeks a pharmacist to manage medicines and improve patient care within a primary care environment. The post holder will work independently the majority of the timewith support from the Clinical Director and GPs within the Peak Edge PrimaryCare Network. Engaging with GP Partners and Practice Managers across the 4Practices covering the Peak Edge Network

This role is pivotal to improvingthe quality of care and operational efficiencies so requires motivation andpassion to deliver an excellent service within general practice.
The postholder will be supported to develop their role to become a non-medical prescriberif that qualification is not already held.
This role will be part of Peak Edge Primary Care Network.
You will be part of the established pharmacy team.
Develop and manage a medicinesmanagement plan and deliver patient services as determined by practice/PCNpolicy and local and national guidance.
Develop and manage the medicinesmanagement team, including delivery of training, to maximise cost-effectiveprescribing and improve the quality of patient care.
Work within their clinical competenciesas part of a multi-disciplinary team to provide expertise in clinical medicinesmanagement, provide face to face structured medication reviews, manage longterm conditions, management of medicines on transfer of care and systems forsafer prescribing, manage repeat prescription authorisations andreauthorisation, acute prescription request, while addressing both the publichealth and social care needs of patients in the GP practice(s) that make up thePCN
Undertake medication review of patientswith polypharmacy especially for older people, people in residential care homesand those with multiple co-morbidities.
Provide leadership on qualityimprovement and clinical audit and well as managing some aspects of the Qualityand Outcomes Framework (QOF)
Provide medication review services to patients via clinics in thepractice, domiciliary visits and in residential and nursing homes, and todeliver pharmaceutical care plans that maximise cost-effective prescribing andimprove the quality of patient care.
Present at patient group meetings or other appropriate events togive advice on the appropriate use of medicines.
Produce patient information leaflets and posters and run medicineawareness projects throughout the year.
Assist partners with the appropriate monitoring and management oftheir prescribing budgets.
Prepare evidence-based resources and information to support themedicine management team and all other relevant health professionals in theimplementation of rational cost-effective prescribing.
Help plan, develop and support the introduction of new workingprocesses within the practice to optimise the quality of prescribing
See (where appropriate) patients with single or multiple medicalproblems where medicine optimisation is required (e.g., Respiratory,Cardiovascular and Diabetes)
Review the on-going need for each medicine, a review of monitoringneeds and an opportunity to support patients with their medicines taking ensuringthey get the best use of their medicines (i.e., medicines optimisation)
Make appropriate recommendations to Senior Pharmacists or GPs formedicine improvement.
Undertake clinical medication reviews with patients and producerecommendations for senior clinical pharmacist, nurses and/or GP on prescribingand monitoring.
Work with care homestaff to improve safety of medicines ordering and administration
