We have had our own Clinical Pharmacy team for several years. You will be working alongside our 3 Clinical Pharmacists and 3 Pharmacy Technicians, be based at our purpose built surgery in Sandbach, and will be an integral part of the clinical team.
We are looking to cover a 20 hour post ideally over 4 days, however this could be negotiable. The interviews are to be held on Thursday 22nd May 2025.
Main duties of the job
You will support the practice with its prescribing, the management of long term conditions, and patients in Care Homes, medical information queries, and discharge medication reconciliation. You will be involved in improving prescribing safety and quality improvement. There will be patient contact, also collaborative working with colleagues across our Primary Care Network.
We are looking for the right person to join us, so would consider part or full-time. Primary care experience is preferred, but not essential. We are keen for the post holder to have a non-medical prescribing qualification, but if this is not held, training can be provided and will be reflected in the starting salary. You will work closely with the Senior Clinical Pharmacist, so will enable mentoring and on the job training.

Maintasks/overview of responsibilities:
Responsibilityfor Patient Care
- Towork as part of the practice multidisciplinary team in developing andimplementing pharmaceutical care plans for patients with long termconditions
Todevelop and implement a programme of care to prevent unnecessary medicationrelated admissions to hospital
Toconduct medication reviews, including monitoring patients clinical results andensuring notes are correct and up-to-date, ensuring that drug safety recommendationsare implemented
- Tosupport and assist Sandbach GPs in achieving safe, evidence-based andcost-effective prescribing of medicines to patients, including supporting delivery of prescribing incentive schemes
To reconcile medicationpost discharge and ensure that communication about medicine changes iseffective
To review medication fornew patients to ensure safe, cost-effective prescribing in line with practice/localprescribing policies
- Toeducate and inform patients about their medicines and provide writteninformation when necessary
Toprovide evidence based advice on prescribing matters to the practice populationin an appropriate form
- Toliaise with pharmacists and other health care professionals in secondaryand primary care to ensure continuity of care

Communicationsand leadership
- Toprovide professional guidance, leadership and support to all staff in thedevelopment and maintenance of evidence-based and cost-effectiveprescribing practice
- Toprovide regular reports and feedback to the lead GP and AshfieldsPartnership Board, on progress with specific plans, budget expenditure andallocation of time to each element of the role
- Toprovide clinical and therapeutic advice to all members of the primaryhealth care team in order to promote safe and effective use of medicines

Partnershipworking
- Towork in partnership with the Senior Clinical Pharmacist, GP Partnerresponsible for Medicines Management,Clinical Pharmacists, and Primary Care Network

Qualifications
- Registration with General Pharmaceutical Council
- Hold or be working towards a Postgraduate Diploma
- Hold or be working towards a prescribing qualification
- Membership of the Royal Pharmaceutical Society

- Minimum 2 years post-qualification experience in hospital, community or primary care pharmacy
- Experience of working within multi-disciplinary teams
