Job description

We have had our own Clinical Pharmacy team for several years. You will be working alongside our 3 Clinical Pharmacists and 3 Pharmacy Technicians, be based at our purpose built surgery in Sandbach, and will be an integral part of the clinical team.

We are looking to cover a 20 hour post ideally over 4 days, however this could be negotiable. The interviews are to be held on Thursday 22nd May 2025.

Main duties of the job

You will support the practice with its prescribing, the management of long term conditions, and patients in Care Homes, medical information queries, and discharge medication reconciliation. You will be involved in improving prescribing safety and quality improvement. There will be patient contact, also collaborative working with colleagues across our Primary Care Network.

We are looking for the right person to join us, so would consider part or full-time. Primary care experience is preferred, but not essential. We are keen for the post holder to have a non-medical prescribing qualification, but if this is not held, training can be provided and will be reflected in the starting salary. You will work closely with the Senior Clinical Pharmacist, so will enable mentoring and on the job training.

Job responsibilities

Mainpurpose of job:

To contribute toimproved health outcomes in long term conditions as a member of themultidisciplinary primary care team

To provide specialistknowledge and advice on pharmaceutical matters, including cost-effectiveprescribing and clinical guidelines for disease treatment, and to also provideevidence-based education and training in partnership with Sandbach GPs

To practice as an independentnon-medical prescriber by holding face to face consultations

Maintasks/overview of responsibilities:

Responsibilityfor Patient Care

• Towork as part of the practice multidisciplinary team in developing andimplementing pharmaceutical care plans for patients with long termconditions

Todevelop and implement a programme of care to prevent unnecessary medicationrelated admissions to hospital

Toconduct medication reviews, including monitoring patients clinical results andensuring notes are correct and up-to-date, ensuring that drug safety recommendationsare implemented

• Tosupport and assist Sandbach GPs in achieving safe, evidence-based andcost-effective prescribing of medicines to patients, including supporting delivery of prescribing incentive schemes

To reconcile medicationpost discharge and ensure that communication about medicine changes iseffective

To review medication fornew patients to ensure safe, cost-effective prescribing in line with practice/localprescribing policies

• Toeducate and inform patients about their medicines and provide writteninformation when necessary

Toprovide evidence based advice on prescribing matters to the practice populationin an appropriate form

• Toliaise with pharmacists and other health care professionals in secondaryand primary care to ensure continuity of care
• Toprovide face to face consultations for long term condition clinics as anindependent non-medical prescriber
• Toassist the practice in responding to patient complaints/conflicts relatedto medicines
• Toundertake clinical audit of care for both medicines and non-medicinesrelated areas.

Responsibilitiesfor Human Resources

• Toprovide evidence-based education and training for Sandbach GPs and staff,involved with the use of medicines
• Tosupport the practice Prescriptions Team by providing training asnecessary.

Responsibilitiesfor physical and financial resources

• Toassist in the management of the practice prescribing budget by ensuringthat prescribing is evidence-based and cost-effective
• Tohelp plan, develop and support the introduction of new working processesto optimise the quality of prescribing for patients and to make moreefficient use of resources (both staff time and financial)
• Toensure the safe and secure handling of medicines in the primary caresetting and that this complies with current legislation.

Communicationsand leadership

• Toprovide professional guidance, leadership and support to all staff in thedevelopment and maintenance of evidence-based and cost-effectiveprescribing practice
• Toprovide regular reports and feedback to the lead GP and AshfieldsPartnership Board, on progress with specific plans, budget expenditure andallocation of time to each element of the role
• Toprovide clinical and therapeutic advice to all members of the primaryhealth care team in order to promote safe and effective use of medicines
• Toliaise with other health care professionals within the multi-disciplinaryteam at Ashfields Primary Care centre
• Toattend/lead (as appropriate) the regular practice meetings
• To empowerothers through knowledge, skills and expertise.

Planningand organising

• Todevelop the necessary strategies, plans and networks required forsuccessful implementation of the practice prescribing policy
• Tolead the implementation of the clinical governance agenda, and NICEguidelines
• Tocomply with and be involved in producing, reviewing and updatingorganisational policies, procedures and guidelines relating to prescribing
• Tobe aware of clinical research and evidence in relation to non-medicalprescribing
• Tohelp ensure extended prescribing rights to nurse practitioners and otherhealth professionals are introduced safely and effectively
• Tomonitor trends and changes in practice and produce reports as deemedappropriate
• Toreview and critically analyse literature, to establish its validity anddisseminate as appropriate
• Todevelop and implement therapeutic guidelines and to facilitate theseprocess both clinically and managerially
• Tobe available to visit care homes when necessary

Partnershipworking

• Towork in partnership with the Senior Clinical Pharmacist, GP Partnerresponsible for Medicines Management,Clinical Pharmacists, and Primary Care Network
• Towork in partnership with managers, multi-professional team, the medicinesmanagement team and non-medical teams to ensure compliance with theprescribing policies and related documents
• Toactively support case managers and other members of the practice multi-disciplinaryteams in medication review and non-medical prescribing matters
• Toprovide clinical and therapeutic advice to all members of the primaryhealth care team in order to promote safe and effective use of medicines

Analysisand data management

• Topromote / research evidence-based practice that reflects the principles ofgovernance and clinical effectiveness to improve the quality of patientcare
• Towork with the appropriate managers and staff to develop systems for riskmanagement and error / near miss reporting for all processes involving theuse of medicines
• Tomaintain professional accreditation and Continuing ProfessionalDevelopment (CPD) as required by the GPhC.

Research, Development and Audit

• Toco-ordinate the evaluation and audit of current prescribing practice andthe impact of change
• Toidentify areas for research or audit and develop research protocols andpolicies
• Toimplement and audit standards in relation to non-medical prescribing in linewith government legislation
• Touse research and audit to identify education and training needs and areasfor service development
• Todisseminate findings from research audits and evaluation.

Person Specification

Qualifications

• Registration with General Pharmaceutical Council
• Hold or be working towards a Postgraduate Diploma
• Hold or be working towards a prescribing qualification
• Membership of the Royal Pharmaceutical Society

Experience

• Minimum 2 years post-qualification experience in hospital, community or primary care pharmacy
• Experience of working within multi-disciplinary teams
• Experience of working in primary care
