Job description

We are seeking a senior clinical pharmacist with experience of working wothin General Practice. You will have completed the CPPE and will be an independent prescriber.TheClinical Pharmacist will have a key role in supporting the delivery of serviceswithin the practice both at practice and network level. For the new StructuredMedications Review and Optimisation requirements this will include tacklingover-medication of patients including inappropriate use of antibiotics,withdrawing medicines no longer needed through NHS-led programmes such as lowpriority prescribing and medicines optimisation more widely. For EnhancedHealth in Care Homes residents will benefit from regular clinical pharmacy-ledmedicines reviews.

Main duties of the job
  • Work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.
  • Take responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage
  • responsibility for day-to-day medication reviews
  • delivering prescribing aspects of the Investment and Impact Fund.
  • Provide support to the practice repeat prescribing systems and medicines reconciliation. Support the implementation of MHRA
  • support patients to take their medications to get the best from them, reduce waste and promote self-care
  • Develop relationships and work closely with other pharmacists across networks and within our federation.
  • Take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists
  • Support CQC compliance.
  • Provide specialist expertise in the use of medicines while helping to address both the public health and social care needs of patients in the network.
  • To complete QUIPP work and other practice-based projects in line with set guidance from the ICB Medicine management team.
  • Proficient in actioning Docman medication addition/amendment tasks.
  • Promote eRD and work to setting this up for the patient population.

Job responsibilities

The Clinical Pharmacist will:

  • Work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.
  • Uphold a prescribing qualification and use this as an independent prescriber.
  • Have completed Centre for Pharmacy Postgraduate Education (CPPE).
  • Take responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple long-term conditions and people with learning disabilities or autism (through STOMP - Stop Over Medication Programme).
  • You will take responsibility for day-to-day medication reviews of patients, some of whom will be on multiple medications.
  • Support the practice with delivering prescribing aspects of the Investment and Impact Fund.
  • Provide support to the practice repeat prescribing systems and medicines reconciliation. Support the implementation of MHRA and other safety alerts at practice level.
  • Provide guidance on person centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, while contributing to the quality and outcomes framework and enhanced services.
  • Through structured medication reviews, support patients to take their medications to get the best from them, reduce waste and promote self-care.
  • Support further integration with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload.
  • Develop relationships and work closely with other pharmacists across networks and within our federation.
  • Take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists (including mental health and reduction of inappropriate antipsychotic use in people with learning difficulties) and anticoagulation.
  • Support CQC compliance.
  • Provide specialist expertise in the use of medicines while helping to address both the public health and social care needs of patients in the network.
  • To complete QUIPP work and other practice-based projects in line with set guidance from the ICB Medicine management team.
  • Proficient in actioning Docman medication addition/amendment tasks.
  • Answer questions from the non-clinical members of the prescribing team.
  • Help where needed in signing off global repeat prescriptions.
  • Promote eRD and work to setting this up for the patient population.
