An NHS GP Practice in Redcar is seeking a Clinical Pharmacist to join their dedicated team. The role involves conducting clinical medication reviews, managing prescriptions, and optimizing patient care through effective medication management. JOB SUMMARY
As a Clinical Pharmacist, the post holder will work as partof a multidisciplinary team to develop and run processes for repeatprescription reauthorisation, deprescribing, management of medicines including transferof care and systems for safer prescribing.
They will work directly with patients to assess and treatconditions, manage long-term conditions as well as promote self-care.They willbe part of a group of Primary Care Network (PCN) Clinical Pharmacists workingunder the supervision and mentoring of a Clinical Pharmacist Manager.
The post holder will be an integral part of the generalpractice team, as well as part of a wider pharmacist network and multi-disciplinaryteam. This includes being a conduit ofpatient medicines information into and out of hospitals. They will work to optimise medication issuesto improve patient care and safety and support clinical staff in the managementof patients.
They will be expected to complete the accredited 18-monthPrimary Care Pharmacy Education Pathway provided by CPPE as part of theirtraining and the support to obtain their Independent PrescriberQualification.
The Green House Surgerycurrently has a registered patient list of approximately 10,000 patientsresiding in Redcar and surrounding areas.
We work from modern,purpose-built premises and benefit from ease of access to hospital-baseddiagnostics and other services. We have excellent public transport links(Redcar train stations are within walking distance) and have ample free on-siteparking for both patients and staff.
Our existing clinical team consists of 4 GP Partners, SalariedGP, Paramedic Practitioner, Nurse Practitioners, Practice Nurses, AdvancedHCAs, HCAs, Phlebotomists and Pharmacy Technicians.
As a member of Redcar CostalPCN, we benefit from having access to various ARRS practitioners and have goodworking relationships with other member practices.
Our excellent PracticeAdministrators and Management team provide nonclinical support and help reducethe administration burden on clinicians.

PrimaryResponsibilities
Patientfacing Clinical Medication Review
Undertake clinical medication reviews with patients withinthe practice, care homes and/or domiciliary visit to proactively managepolypharmacy and produce recommendations for senior clinical pharmacist, nursesand/or GP on prescribing and monitoring.
See (where appropriate) patients with single or multiplemedical problems where medicine optimisation is required (e.g. Respiratory,Cardiovascular and Diabetes).
Clinics are done face to face, via telephone, home visitsand in the future, possibly video consultations.
The post holder will undertake clinics for patients ordevelop one to one relationship in order to deprescribe where necessary oroffer clinical leadership on medicines optimisation and quality improvement.
Review the on-going need for each medicine, a review ofmonitoring needs and an opportunity to support patients with their medicineadherence ensuring they get the best use of their medicines (i.e. medicinesoptimisation). Make appropriate recommendations to GPs for medicineimprovement.
Patientfacing Care Home and Domiciliary Clinical Medication Reviews
Undertake clinical medication reviews with patients andproduce recommendations for clinicians on prescribing and monitoring. Work withcare home staff to improve safety of medicines ordering and administration.
Managementof common/minor/self- limiting ailments
Managing caseload of patients withcommon/minor/self-limiting ailments while working within a scope of practiceand limits of competence.
Signposting to community pharmacy and referring to GPs orother healthcare professionals where appropriate.
Provide patient facing clinics and a telephone helpline forthose with questions, queries, and concerns about their medicines in thepractice.
Medicineinformation to practice staff and patients
Answers relevant medicine-related enquiries from GPs, otherpractice staff, other healthcare teams (e.g. community pharmacy) and patientswith queries about medicines.
Suggesting and recommending solutions.
Providing follow up for patients to monitor the effect ofany changes
Telephone medicines support
Provide a telephone help line for patients with questions, queries,and concerns about their medicines.
Helpingtackle the issue of unplanned hospital admissions
Review the use of medicines most associated with unplannedhospital admissions and re-admissions through audit and individual patientreviews.
Put in place changes to reduce the prescribing of thesemedicines to high-risk patient groups.
Managementof medicines at discharge from hospital
To reconcile medicines following discharge from hospitals,intermediate care and into care homes, including identifying and rectifyingunexplained changes and working with patients and community pharmacists toensure patients receive the medicines they need post discharge.
Set up and manage systems to ensure continuity of medicinessupply to high-risk groups of patients (e.g. those with medicine complianceaids or those in Care Homes).
Signposting
Ensure that patients are referred to the appropriatehealthcare professional for the appropriate level of care within an appropriateperiod e.g. pathology results, common/minor ailments, acute conditions, longterm condition reviews etc.
Repeatprescribing
Produce and implement a repeat prescribing policy. Manage the repeat prescribing reauthorisation process byreviewing patient requests for repeat prescriptions and reviewing medicinesreaching review dates and flagging up those needing a review.
Ensure patients have appropriate monitoring tests in placewhen required.
Riskstratification
Identification of cohorts of patients at high risk of harmfrom medicines through pre-prepared practice computer searches. This mightinclude risks that are patient related, medicine related, or both.
Servicedevelopment
Contribute pharmaceutical advice for the development andimplementation of new services that have medicinal components (e.g. advice ontreatment pathways and patient information leaflets).
Medicinesquality improvement
Undertake clinical audits of prescribing in areas directedby the GPs, feedback the results and implement changes in conjunction with therelevant practice team.
Implement changes to medicines that result from MHRA alerts,product withdrawal and other local and national guidance.
Auditing practices compliance against NICE technologyassessment guidance.
CareQuality Commission
Work with the general practice team to ensure the practiceis compliant with CQC standards where medicines are involved.
CollaborativeWorking Relationships
Participates in the Primary Care Network MultidisciplinaryTeam.
Liaises with ICB colleagues including ICB pharmacists onprescribing related matters to ensure consistency of patient care and benefitin surgeries.
Foster and maintain strong links with all services acrossthe PCN and neighbouring networks.
Explores the potential for collaborative working and takesopportunities to initiate and sustain such relationships.
Recognises the roles of other colleagues within theorganisation and their role to patient care.
Demonstrates use of appropriate communication to gain theco-operation of relevant stakeholders (includingpatients, senior and peer colleagues, and otherprofessionals, other NHS/private organisations e.g. ICBs).
Can recognise personal limitations and refer to moreappropriate colleague(s) when necessary.
Demonstrates ability to integrate general practice withcommunity and hospital pharmacy teams.
Liaises with other stakeholders as needed for the collectivebenefit of patients including but not limited to:
Patients and their representatives.
GP, nurses, and other practice staff.
Other healthcare professionals includingcommunity pharmacists, pharmacy technicians, social prescribers, first contactphysiotherapists, physicians associate and paramedics.

Other members of the medicines management (MM)team including pharmacists, technicians, and dieticians

Community nurses and other allied health professionals
Hospital staff with responsibilities forprescribing and medicines optimisation.
ProfessionalDevelopment
Work with your line manager to undertake continual personaland professional development, taking an active part in reviewing and developingthe role and responsibilities.
Involved in one-to-one meetings with line manager monthly todiscuss targets and outcomes achieved.
Adhere to organisational policies and procedures, includingconfidentiality, safeguarding, lone working, information governance, and healthand safety.
Work with your line manager to access regular clinicalsupervision, to enable you to deal effectively with the difficult issues thatpeople present.
Review yearly progress and develop clear plans to achieveresults within priorities set by others.
Participate in the delivery of formal education programmes.
Demonstrate an understanding of current educational policiesrelevant to working areas of practice and keep up to date with relevantclinical practice.
Knowledge,Skills and Experience Required:
Completion of an undergraduate degree in pharmacy andregistration with the General Pharmaceutical Council.
Minimum of 2 years experience as a pharmacist.
You will be expected to complete the accredited 18 month Primary care Pharmacy Education Pathway provided by CPPE.
Hold independent prescribing qualification or willingness to work towards.
Have experience and an awareness of common acute andlong-term conditions that are likely to be seen in general practice.
Recognises priorities when problem-solving and identifiesdeviations from normal pattern and can refer to seniors or GPs whenappropriate.
Able to follow legal, ethical, professional, andorganisational policies/procedures and codes of conduct.
Involves patients in decisions about prescribed medicinesand supporting adherence as per NICE guidelines.
The practices use the clinical system software systmone andEMIS.
