Job description

We are seeking a Clinical Pharmacist to join our healthcare team. The ideal candidate will provide expert pharmaceutical care to patients in a clinical setting, focusing on patient care and medication administration. The job role will be required to be working at Kingsfold Medical Centre.

Main duties of the job
  1. To carry out medication reviews face to face or remote and proactively manage patients (some with complex polypharmacy). This will include patient education, prescribing, and monitoring as per guidelines and within professional competencies.
  2. To carry out face-to-face and telephone consultations with patients for Structured Medication Reviews.
  3. Work closely with the wider team, advising and supporting on medicine management issues.
  4. Undertake prescribing audits.
  5. Ensure that the practice is encouraged to promote cost-effective prescribing.
  6. Ensure medicines are prescribed and monitored as per the most current guidelines.
  7. Ensure that the practice systems for drug safety and drug safety monitoring are adhered to at all times. This work will include regular safety audits.
  8. To reauthorise medication for repeat prescribing within scope of practice. Identify patients suitable for repeat prescribing, liaising with the relevant clinicians as required.
  9. Review and implement safe prescribing systems for vulnerable patients, improving compliance support clinicians with the management of patients suffering from drug and alcohol dependencies.
  10. Share knowledge and learning with the wider teams.
  11. Attend regular practice meetings.
  12. Represent the practice at outside events.
  13. Contribute to achieving high IIF/QOF targets.
  14. Undertake medication reviews for housebound/care home patients as needed.
  15. Take an active role to help/support practice initiatives.
About us

We have a current list size of 5100 patients, two partners (one clinical and one non-clinical), 2 salaried GPs, and 2 part-time practice nurses. We are part of Greater Preston network Group, which comprises ten practices. We have a practice manager, Deputy PM, Admin, Reception, and Secretarial team.

Additional roles include GP Assistant, Physician associate, clinical pharmacist, social prescriber, and Advanced Clinical practitioner.
