Job Summary
We are looking for a dedicated and experienced
Clinical Pharmacist to join our Primary Care Network (PCN), working across
three GP practices to enhance the quality and safety of prescribing and medicines management.
This Post Is Integral To The NHSE Future Plan For General Practice Which Encourages The Development Of This Role And Will Partake In The National Learning And Knowledge Events. We Are Seeking Individuals To Lead The Development Of This Role During An Exciting Time For General Practice. Within These Positions You Will Directly Improve Patient Care And Support The Efficient Running Of a New Model Of General Practice. You Will Become The Practice Expert In Medicines Optimisation And Have Input Into The Following Areas But Not Limited To:
undertaking structured medication reviews,
improving medicines optimisation and safety
supporting care homes
running practice clinics
You will develop through team clinical supervision sessions and benefit from direct mentoring with an experienced GP/ Clinical Lead. You will have the opportunity to access and complete NHS Englands accredited training pathway that equips you to practice and prescribe safely and effectively in a primary care setting.
Main duties of the job
Key Responsibilities
- Deliver high-quality clinical pharmacy services across three GP practices in the PCN.
- Provide medication reviews, manage long-term conditions, and support repeat prescribing processes.
- Promote evidence-based prescribing and advise on cost-effective medicine use.
- Support clinicians with complex prescribing decisions and therapeutic queries.
- Contribute to the development of prescribing policies and local guidelines.
- Communicate clearly and sensitively with patients and clinical teams.
- Work collaboratively with GPs, nurses, and other allied health professionals to deliver integrated care.
About Us
The Brunel Primary Care Network consists of three Practices across Portsmouth; East Shore Partnership. The Lighthouse Group and University Surgery.
We have highly skilled and motivated teams working across our PCN, which consist of GP Partners, Salaried GP's, Advanced Nurse Practitioners, Pharmacists, Diabetes and Respiratory Nurse Specialists, Nurses and HCA's; who are supported by skilled administration teams.

The post holder will provide leadership on quality improvement and clinical audit, as well as managing some aspects of the Quality and Outcomes Framework.
This role is pivotal to improving the quality of care and operational efficiencies so requires good motivation and passion to deliver an excellent service within a General Practice environment.

- Meet DBS reference standards and has a clear criminal record, in line with the law on spent convictions.
- Adaptable and self motivated.
- Access to own transport and ability to travel across the locality on a regular basis including visiting people in their own homes.

- Excellent interpersonal, influencing and negotiating skills.
- Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
- Gain acceptance for recommendations and influence, motivate, persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers.

Essential
- Strong therapeutic and clinical knowledge with a firm understanding of evidence-based healthcare.
- Insight into the workings of general practice and the primary care environment.
- Appreciation of the nature of primary care prescribing, concepts of rationale prescribing and strategies for improving prescribing.
- Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients)
Qualifications

- Registered with the General Pharmaceutical Council (GPhC).
- Minimum of 5 years post-qualification experience in a clinical setting.
- Undertaking, completed or can demonstrate exemption from CPPE's Primary Care Pharmacy Education Programme.
- Minimum of 5 years post - qualification experience.

- Independent prescriber of currently working towards qualification.
