Job summary

Are you an experienced Clinical Pharmacist who is proactive, enthusiastic and looking for a positive challenge and an opportunity to provide services in an area where patients need it the most? Then this could be the role for you.

This role will report to the Clinical Lead and Practice Manager, working alongside a brilliant team of Doctors, Nurses, HCAs and Administrative staff who deliver great healthcare to our patients.

Main duties of the job

  1. Undertake medication reviews with a diverse group of patients, discuss results with patients, carry our HRT and contraception reviews.
  2. Set up and manage systems to ensure continuity of medicines supply to high-risk groups of patients (eg, those with medicine compliance aids or those in care homes).
  3. Identification of cohorts of patients at high risk of harm from medicines through pre-prepared practice computer searches. This might include risks that are patient related, medicine related, or both.
  4. Manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates.
  5. Monitor and manage patients with polypharmacy.

Job responsibilities

  1. Expertise in clinical medicines review and address public health and social needs of patients in GP practices.
  2. Reduce inappropriate poly pharmacy and wasteful prescribing through clinical medication review.
  3. Reconcile medicines following hospital discharge and work with patients and community pharmacists to ensure patients receive the medicines they need post discharge.
  4. Contribute to reductions in medicine related hospital admissions and readmissions by supporting patients to get the best outcomes from their medicines and identifying and addressing medicines related issues.
  5. Manage the repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions and medicines reaching review dates.
  6. Manage patients and health care professionals medicine queries.
  7. Increase quality and safety of prescribing through mechanisms such as audit and PDSA cycles.
  8. Implement drug withdrawals and alerts e.g. MHRA aimed at improving medicines safety.
  9. Work with primary care professional and patients to implement NICE and other evidence based guidelines.
  10. Run clinics where medicines are the main interventions.
  11. Contribute to multimorbidity reviews and multidisciplinary reviews.
  12. Provide medicines information and training to Practice healthcare professionals and admin staff.
  13. Review daily Pathology results for patients on known medicines.
  14. Act as a source of medicines information for all of the practice team and patients (e.g. around doses, side effects, adverse events, possible alternatives e.g. around out of stocks).
  15. Undertake minor ailments triage: dealing with minor ailments and triaging patients appropriately.

CORE RESPONSIBILITIES

  1. Working within the practice based team to undertake medication reviews particularly in high risk groups such as: Frail elderly, Poly-pharmacy, Renal impairment, Hepatic impairment, Substance misuse, Patients on high risk medicines, STOPP/START identified patients, Revolving door Hospital admissions.
  2. Improve prescribing practice through educational support for all prescribers within the practice.
  3. Lead on where changes in evidence require changes in prescribing across patient population e.g. where a drug is withdrawn or indications change.
  4. Liaise with colleagues in community pharmacy to align support for medicines adherence such as MUR and NMS.
  5. Support improvements in clinical care through practice based audit and implementing change.
  6. Prescribing advice to prescribers in practice e.g. temporary non availability of drugs.
  7. Ensure patient safety when they are transferred between care providers through reconciliation of prescribed medicines.

Person Specification

Qualifications

Essential

  • Completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council.
  • Completed the CPPE primary care pathway or be working towards it or willing to work towards it.

Desirable

  • Independent Prescribing Qualification.
  • Master's degree in pharmacy (MPharm).

Experience

Essential

  • Understanding of the work of General Practice.
  • Drive and determination to promote and support the role of pharmacy staff in General Practice.
  • Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate.
  • Demonstrates the ability to communicate complex and sensitive information in an understandable form to a variety of audiences.
  • Is able to plan, manage, monitor, advise and review general medicine issues in core areas for long term conditions.
  • Able to work under pressure and to meet deadlines.
  • Work effectively independently and as a team member.
  • Understands the principles of equality and diversity and commitment to addressing health inequalities and patient empowerment.
  • Experience of providing and using information to support the delivery and performance management of services.
  • Ability to communicate effectively to a wide range of audiences, both written and verbal.
  • Ability to demonstrate personal professional resilience and seek advice when needed.
  • Ability to travel to various locations within the locality.
  • An Enhanced DBS check is required for the role.

Desirable

  • An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
  • Able to obtain and analyse complex technical information.
  • Experience in the use of various IT applications and the recording of clinical notes (EMISWeb, Docman).
