We are looking for Clinical Pharmacist to work with ClaremontClinic.

***PART-TIME ROLE, IDEALLY WEDNESDAY TO FRIDAY, 9AM TO 5PM WITH 30 MINUTES UNPAID LUNCH (22.5 HOURS)***

We are looking for an enthusiastic, motivated experiencedgeneral practice prescribing pharmacist who is committed to developingknowledge and skills to improve patient care in general practice.

You will be responsible for their clinical competencies aspart of a multi-disciplinary team to provide expertise in clinical medicinesmanagement, provide face to face medication reviews, manage long termconditions, management of medicines on transfer of care and systems for saferprescribing, manage repeat prescription authorisations and reauthorisation,acute prescription request, while addressing both the public health and socialcare needs of patients in the GP practice.

Main duties of the job

Core Responsibilities:

  • Medication Reviews:Conduct structured medication reviews to ensure safe, effective, and appropriate use of medicinesespecially for patients with long-term conditions or those on multiple medications.
  • Prescribing & Deprescribing:Initiate, adjust, or stop medications under supervision or as an independent prescriber.
  • Repeat Prescription Management:Oversee repeat prescriptions, checking for drug interactions, duplications, and adherence issues.
  • Support for Chronic Disease Management:Work alongside GPs and nurses to manage conditions like hypertension, diabetes, asthma, and COPD.
  • Patient Consultations:Provide face-to-face or telephone reviews to discuss medication concerns, side effects, or lifestyle advice.
  • Clinical Audits & Safety Monitoring:Monitor prescribing trends, support audits, and promote safe prescribing practices.
  • Staff Support & Education:Act as a resource for other healthcare staff, offering advice on medications and contributing to in-practice training.

This role reduces GP workload, improves patient outcomes, and enhances prescribing safety across the practice.

Job responsibilities

  • Work as part of a multi-disciplinary team in apatient facing role to clinically assess and treat patients using their expertknowledge of medicines for specific disease areas
  • Be responsible for the care management ofpatients with chronic diseases undertake clinical medication reviews toproactively manage people with complex polypharmacy, especially the elderly,people in care homes, those with multiple co-morbidities (frailty, COPD andasthma) and people with learning disabilities or autism
  • Provide specialist expertise in the use ofmedicines whilst helping to address both the public health and social careneeds of patients at the PCNs practice(s) and to help in tackling inequalities
  • Provide leadership on person-centred medicinesoptimisation (including ensuring prescribers in the practice conserveantibiotics in line with local antimicrobial stewardship guidance) and qualityimprovement, whilst contributing to the quality and outcomes framework andenhanced services
  • Through structured medication reviews, supportpatients to take their medications to get the best from them, reduce waste andpromote self-care
  • Have a role in supporting further integration ofgeneral practice with the wider healthcare teams (including community andhospital pharmacy) to help improve patient outcomes, ensure better access tohealthcare and help manage general practice workload
  • Develop relationships and work closely withother pharmacy professionals across PCNs and the wider health and social caresystem
  • Take a central role in the clinical aspects ofshared care protocols, clinical research with medicines, liaison withspecialist pharmacists (including mental health and reduction of inappropriateantipsychotic use in people with learning difficulties), liaison with communitypharmacists and anticoagulation
  • Be part of a professional clinical network andhave access to appropriate clinical supervision
  • High risk drug monitoring, and involved inaudits, set by CCG or towards borough wide aims and targets. Run searches forlocal and national medication alerts/shortages.

LTC Management

  • See (where appropriate) patients with single ormultiple medical problems where medicine optimisation is required (e.g. COPD,asthma).
  • Review the on-going need for each medicine, areview of monitoring needs and an opportunity to support patients with theirmedicines taking ensuring they get the best use of their medicines (i.e.medicines optimisation)
  • Make appropriate recommendations to SeniorPharmacists or GPs for medicine improvement

Management of common/minor/self-limiting ailments

  • Managing caseload of patients withcommon/minor/self-limiting ailments while working within a scope of practiceand limits of competence.
  • Signposting to community pharmacy and referringto GPs or other healthcare professionals where appropriate Patient facingmedicines support

Medicines support to patients

  • Provide patient clinics for those withquestions, queries and concerns about their medicines in the practice
  • To reconcile medicines following discharge fromhospitals, intermediate care and into care homes, including identifying andrectifying unexplained changes and working with patients and community
  • Pharmacists to ensure patients receive themedicines they need post discharge
  • Set up and manage systems to ensure continuityof medicines supply to high risk groups of patients (e.g.those with medicine compliance aids or those in care homes).

Medicines support to practice staff

  • Answers relevant medicine-relatedenquiries from GPs, other practice staff, other healthcare teams (e.g.community pharmacy) and patients with queries about medicines.
  • Suggesting and recommending solutions.
  • Providing follow up for patients to monitor theeffect of any changes
  • Do appropriate database searches, in keepingMHRA and NICE alerts/ guidance to support safe patient care.

Signposting

  • Ensure that patients are referred to theappropriate healthcare professional for the appropriate level of care within anappropriate period e.g. pathology results, common/minor ailments, acuteconditions, long term condition reviews etc

Repeat prescribing

  • Produce and implement a practice repeatprescribing policy.
  • Manage the repeat prescribing reauthorisationprocess by reviewing patient requests for repeat prescriptions and reviewingmedicines reaching review dates and flagging up those needing a review.
  • Ensure patients have appropriate monitoringtests in place when required.
  • Identification of cohorts of patients at highrisk of harm from medicines through pre-prepared practice computer searches.
  • This might include risks that are patientrelated, medicine related, or both
  • Contribute pharmaceutical advice for thedevelopment and implementation of new services that have medicinal components(e.g. advice on treatment pathways and patient information leaflets
  • Comply with local medicines managementrequirements.
  • Undertake clinical audits of prescribing inareas directed by the GPs, feedback the results and implement changes inconjunction with the practice team.
  • Present audit findings to improve servicedelivery as appropriate.
  • Implement changes to medicines that result fromMHRA alerts, product withdrawal and other local and national guidance

Care Quality Commission

  • Work with the general practice team to ensurethe practice is compliant with CQC standards where medicines are involved.

Public health

  • To support public health campaigns.

Experience
