Job description

Vida Healthcare has an exciting and new opportunity that has arisen. We are currently looking to employ a reliable and hard-working permanent registered pharmacist on a full or part-time basis, to provide additional pharmacy support to the practice.The role will cover work for patients registered across the four sites.

You will be responsible for providing a high-quality service to all our patients, help to achieve prescribing targets within the prescribing team and ensure patient data is accurate. In addition, you will be offering an excellent standard of support to all clinical and admin staff, ensuring good quality service delivery and confidentiality, with patient satisfaction a priority

Main duties of the job
  • Provide pharmaceutical consultations to patients with long term conditions as an integral part of the multidisciplinary team.
  • Draw up structured medication reviews.
  • Complete complex discharge and clinic letters.
  • Improve patient and carer understanding of confidence in and compliance with their medication.
  • Provide advice and answer medication related queries from patients and staff.
  • Organise and oversee the work of the PCN pharmacy team, to help ensure the requirements of the practice are being fulfilled and align.
  • Improve the quality and effectiveness of prescribing through clinical audit and education to improve performance against NICE standards, and clinical and prescribing guidance.
  • Develop yourself and the role through participation in clinical supervision, training, and service redesign activities.
  • Contribute to a patient safety culture through reporting and investigation of medicines incidents and undertaking proactive measures to improve patient safety.
  • Work with community pharmacists, hospital pharmacists, and other stakeholders in the medicines supply chain to improve patient experience and manage incidents.

Qualifications

  • If not already obtained, as part of their employment, the post holder will achieve qualification from an accredited training pathway. (CPPE Clinical Pharmacist Training pathway)
