A prominent healthcare provider in Great Yarmouth is seeking a Clinical Pharmacist to join their practice team. This role involves chronic disease management, clinical medication reviews, and providing leadership on medicines optimisation. This is an exciting opportunity for a motivated and enthusiastic Clinical Pharmacist to work as part of our established practice pharmacy team. As a pharmacy team we work alongside the rest of the practice team and the Gorleston Primary Care Network pharmacy team to provide high-quality, integrated patient care.
The post holder will work as part of a multi-disciplinaryteam in a patient-facing role. The post holder will take responsibility forareas of chronic disease management within the practice and undertake clinicalmedication reviews to proactively manage patients with complex polypharmacy.
The post holder will provide clinical leadership onmedicines optimisation and quality improvement, including managing projects related to prescribing quality and patient safety, as well as managing some aspects of thequality and outcomes framework and enhanced services.
We pride ourselves in offering a friendly, efficient and professional service for approximately 25,000 patients operating over 3 sites, two sites are in Gorleston and one in Hopton.
The practice has a dynamic team of GPs, ANPs, Paramedics, Pharmacists and Pharmacy Technicians, Practice Nurses, HCAs, Phlebotomist, a Managerial team and administrative support staff.
We are a well established teaching practice, involved with GP Registrars, Foundation Year 2 trainees also, an accredited research practice
The practice is an active member of the Gorleston Primary Care Network.
We have been rated as "Good" by CQC.
The post holder will be a pharmacist, who acts within theirprofessional boundaries, supporting and working alongside a multidisciplinary team in general practice. In this role you will be supported by the Partners, Pharmacy team and the wider practice team to develop the role and associated skills.
The post holder will work as part of a team in a patient-facing role. The post holder will take responsibility forareas of chronic disease management within the practice and undertake clinicalmedication reviews to proactively manage patients with complex polypharmacy.
The post holder will provide primary support to generalpractice staff with regards to prescription and medication queries. They willhelp support the repeat prescription system, deal with acute prescriptionrequests, and medicines reconciliation on transfer of care and systems forsafer prescribing, providing expertise in clinical medicines advice whileaddressing both public and social care needs of patients in the GP practice.
The post holder will provide clinical leadership onmedicines optimisation and quality improvement, including managing projects related to prescribing quality and patient safety, as well as managing some aspects of thequality and outcomes framework and enhanced services.Responsible for dealing withMHRA Alerts and any actions these require including historic Alerts.
The post holder will ensure that the practice integrateswith community and hospital pharmacy to help utilise skill mix, improve patientoutcomes, ensure better access to healthcare and help manage workload. The roleis pivotal to improving the quality of care and operational efficiencies sorequires motivation and passion to deliver excellent service within generalpractice.
The post holder willbe supported to develop their role to become a non-medical prescriber if not already completed.
