The ClinicalPharmacist will work as part of a MDT and pharmacy team comprising of otherclinical pharmacists, pharmacy technicians and pharmacy administrators. The post holder must be passionate about deliveringhigh quality patient care, ensuring patient and drug safety, and in providingholistic patient centred care. Theclinical pharmacist will conduct structured medication reviews, LTC reviews,deprescribing where appropriate, manage care home patients and patients withfrailty mediation reviews, support the wider practice team with drug relatedqueries, support drug monitoring processes and drug safety. The clinicalpharmacist will support the lead pharmacist, and with the rest of the pharmacyteam, PCN CD and PCN manager in ensuring that targets around the PCN DES, IIFand practice QOF, drug safety and monitoring searches are met.
The post holder must be passionate about delivering high quality patient care, ensuring patient and drug safety, and in providing holistic patient centred care. The clinical pharmacist will conduct structured medication reviews, support wider practice team with drug related queries, support drug monitoring processes and drug safety.
PCCO PCN has a diverse younger than average patient population with higher levels of deprivation linked with obesity, smoking, mental health, and cardiovascular disease. PCN main practice branch is centred in Normanton with a multicultural population adding challenges of language barriers, health inequalities, and patient understanding of health. PCCO PCN has higher than average use of opiates, and other prescribed drug dependency drugs.
