Job description

We are pleased to offer this exciting opportunity for an enthusiastic, newly qualified or experienced proactive Clinical Pharmacist to join Witterings Medical Centre. You will be joining an established, friendly team of multidisciplinary healthcare professionals to deliver medicines optimisation services, undertake clinical medication reviews to proactively manage people with complex polypharmacy, both face-to-face and on the telephone. Additionally, you will support the practice in providing expert advice with medication queries and monitoring.

Enquiries and informal visits are very welcome.

Main duties of the job
  1. Contribute to the delivery of effective daily processes for the transfer of care between hospitals and GPs, including effective signposting, recommendation of medication changes, and communication accordingly.
  2. Support colleagues and patients with medication queries and monitoring.
  3. Use structured medication reviews to support patients in taking their medications correctly, reduce waste, and promote self-care.
  4. Undertake clinical medication reviews to proactively manage people with complex polypharmacy.
  5. Provide leadership on person-centred medicines optimisation, including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance, and engage in quality improvement.
  6. Take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists, community pharmacists, and anticoagulation clinics.

Job responsibilities
  • Contribute to the effective daily processes for transfer of care between hospitals and GPs, including signposting and communication.
  • Support colleagues and patients with medication queries and monitoring.
  • Use structured medication reviews to support medication adherence, reduce waste, and promote self-care.
  • Undertake clinical medication reviews for patients with complex polypharmacy.
  • Lead person-centred medicines optimisation, including antimicrobial stewardship and quality improvement initiatives.
  • Participate in shared care protocols, clinical research, and liaise with pharmacists and clinics.
  • Contribute to QOF and enhanced services.
  • Support practice compliance with CQC standards regarding medicines.
  • Assist in integrating general practice with other healthcare teams to improve outcomes and access.
  • Develop relationships with pharmacy professionals across Primary Care Networks and the wider health system.
  • Manage care for patients with chronic diseases.
  • Provide pharmaceutical advice for new services involving medicines.
  • Be a prescriber or work towards becoming one.

    Person Specification
    Qualifications
    • Undergraduate degree in pharmacy and registration with the GPC.
    Experience
    • Working towards independent prescribing qualification.
    • Experience with common acute and long-term conditions.
    • Ability to work effectively independently.
    • Excellent interpersonal, influencing, and negotiating skills.
    • Adherence to legal, ethical, professional, and organisational policies.
    • Commitment to continuous professional development.

    • More than 2 years of experience as a pharmacist.
    • Experience in medication reviews relevant to General Practice.
    • Proven track record of improving work processes.
    • Previous experience working in a GP surgery.
    • Knowledge of CQC regulations in General Practice.
    • Experience with quality improvement through audits.
    • Familiarity with TPP SystmOne.
