The successful candidate will work to deliver safe, effective,and cost-effective use of medicines by applying their pharmaceutical, clinicaland technical skills. This role will provide a brilliant opportunity for theright candidate to develop their knowledge and skills. Inaddition, they will be responsible for encouraging the development of betterunderstanding of the principles of medicines optimisation throughout thepractice teams and promoting good practice in line with therapeutic developments.This will involve assisting the PCN in achieving national requirements, NICEimplementation and utilisation of medicines optimisation initiatives.
This role is part ofthe extended hours provision and may include Saturdays on a rota basis.
Provide clinical support to the surgeries within the Bridgwater Bay Primary Care Network as an Independent Pharmacist Prescriber, consulting with patients through face-to-face clinics, telephone appointments and video consultations.,
Provide clinical support topatients in care and nursing homes as well as the wider community.
Carry out structured medicationreviews for all patient cohorts.
Support the PCN practices to improve medicines reconciliation on discharge, formulary compliance, repeatprescribing management and medication reviews.
Support Bridgwater Bay PCN andSomerset ICB plans to reduce medicines waste in primary care.
Bridgwater Primary Care Network(PCN) is the largest PCN in Somerset with 9 GP practices and a diversepopulation spread across town and rural locations.
As a PCN we are forwardthinking, innovate and driven to deliver the best patient care for ourpopulation. This includes health population management, and this role ties inwith supporting that and tracking the improvements we can make to patientslives.
In partnership with Somerset NHS Foundation Trust, we have developed a joint venture to open a Health and Wellbeing Hub at the former Victoria Park Medical Centre. This flagship hub, the first of its kind, brings together Primary and Secondary Care under one roof to provide joined-up services and support for the Bridgwater Bay community.
Key responsibilities
Provide clinical support to the surgeries within the Bridgwater Bay Primary Care Network as an independentpharmacist prescriber, consulting with patients by telephone and videoconsultation,

Interpret and provide complexmedicines information to patients, carers and healthcare professionals.
To provide information, adviceand guidance on the safe, rational, effective and economical use of medicines.
Undertake clinical audit andother outcome measures to support patient care and safety.
Create and implement medicinerelated guidelines, policies and SOPs as appropriate to the different serviceareas.
Provide and receive complex,sensitive information and communication relating to pharmaceutical care of patients.
Provide prescribing servicewithin sphere of competency to patients in primary care, as a pharmacistindependent prescriber.
To develop, advise, agree andimplement action plans at a practice level, monitor the effect of changes madeand ensure that any benefits are maintained in the longer term. This willinclude:
Reviewing prescribing data forthe practices to highlight areas where there is scope for improving theclinical effectiveness of prescribing.
Obtaining agreement with thepractices to implement medicines optimisation projects with the overall aim offollowing national prescribing guidance and cost effective prescribing.
Advising GPs and other healthprofessionals on cost effective prescribing and aiding rationalisation of drugchoice via formulary/guideline/protocol development.
Record clinical activity,interventions, KPIs and to monitor the outcome of interventions.
Provide training on medicines ona regular basis to GP surgery staff, patients and carers in all relevantaspects of medicine use.
Support the pharmacy techniciansworking within the PCN.
To maintain knowledge ofdevelopments in pharmacy practice on a regular basis in line with GeneralPharmaceutical Council (GPhC) CPD requirements
To fulfil the professionalrequirements of the Code of Ethics and Standards of Good professional Practiceof the GPhC
The post holder will beresponsible to the SHS Lead Pharmacist and Primary Care Network Clinical Directorbut will be expected to use their own initiative to make complex decisionswithin defined parameters with little supervision.
Requirement to participate inthe national training scheme for pharmacists in primary care.
