Job Summary

Epsom Primary Care Network (PCN) is seeking a highly experienced and qualified Clinical Pharmacist to join one of our member GP practices on a maternity cover basis. Epsom PCN serves a diverse population of approximately 62,000 patients across six practices. While the successful candidate will be primarily based at a single practice, some flexibility to support other sites may be required.

Please note: Due to the maternity cover nature of this role, we will only consider applicants who have successfully completed the CPPE Primary Care Pharmacy Education Pathway and have a minimum of two years' experience working in general practice.

Our well-established and collaborative pharmacy team is supported by experienced pharmacy technicians and a dedicated care coordinator. With new funding awarded to pilot innovative, transformational healthcare initiatives, this is an exciting opportunity to contribute to a forward-thinking and dynamic team making a meaningful impact in primary care.

Main duties of the job

Clinical Pharmacists in primary care are essential members of the multidisciplinary team, working closely with GPs, nurses, and other healthcare professionals to optimise patient care. Their role is both clinical and strategic, focused on ensuring safe, effective, and evidence-based use of medicines.

  • Medication Reviews and Optimisation
  • Conduct structured medication reviews for patients, particularly those with long-term conditions, polypharmacy, or complex needs.
  • Identify and resolve medication-related problems to improve patient outcomes.
  • Support deprescribing where appropriate, reducing the risk of harm from overmedication.
  • Manage clinics for specific conditions (e.g. hypertension, asthma, diabetes) within their scope of practice.
  • Provide expert advice to patients on the safe and effective use of medications.
  • Monitor patients for side effects, interactions, and therapeutic effectiveness.
  • Review and authorise repeat prescriptions to ensure accuracy and appropriateness.
  • Streamline repeat prescribing systems to improve efficiency and reduce waste.
  • Conduct medicines reconciliation for patients after hospital discharge, ensuring continuity of care and accurate medication records.

Job Purpose

The Clinical Pharmacist will work within their professional boundaries as a key member of the Epsom Primary Care Network (PCN), delivering patient-facing clinical services and medicines optimisation across its member GP practices. This role is central to improving patient outcomes through high-quality prescribing, structured medication reviews, and long-term condition management.

Working closely with GPs, practice teams, and the wider multidisciplinary workforce, the post holder will also contribute to safer prescribing, reduce medicine-related risks, and support service improvement initiatives across the PCN. The successful candidate may be supported to train as an independent prescriber, if not already qualified.

Key Responsibilities

  • Patient-Facing Clinical Services
  • Conduct structured medication reviews for patients with long-term conditions, complex polypharmacy, or identified risks.
  • Manage condition-specific clinics (e.g. asthma, COPD, hypertension) and support disease prevention and health promotion.
  • Provide medicines support in care homes and domiciliary settings, including reconciliation, review, and administration guidance.
  • Offer patient education through face-to-face and telephone consultations, addressing concerns and promoting adherence.
  • Medicines Optimisation & Safety
  • Provide clinical leadership on prescribing quality, safety, and efficiency in line with NHS guidance and PCN priorities.
  • Lead audits, implement MHRA alerts and NICE guidance, and support compliance with local formularies and shared care protocols.
  • Review and authorise repeat prescriptions, ensuring appropriate monitoring and safe prescribing practices.
  • Integrated Care & Service Development
  • Support the transition of care by reconciling medicines post-discharge and minimising medication-related hospital readmissions.
  • Participate in multidisciplinary team meetings, care planning, and case conferences to ensure coordinated patient care.
  • Identify cohorts of high-risk patients using clinical systems and support proactive interventions.
