Clinical Nurse Specialist - Rapid Response Team

Salary: Agenda for Change Band 7 £54,302 - £60,981 pa dependent on experience (includes HCA Supplement)

Hours: Full time (Worked as 150 hours over 4 weeks 12-hourshifts, 8am - 8pm, 7 days/ week including weekends and public holidays)

Length of Tenure: Permanent

We are looking for an experienced specialist palliative care nurse who can operate within a fast paced, dynamic and flexible specialist palliative care rapid response service.

This post is suited to a senior nurse with significant experience of caring for patients with life limiting illnesses who possesses proven expertise in holistic assessment, palliative symptom management and advanced communication.

You will be a key member of the rapid response team, working closely with the wider multidisciplinary team both in the hospice and externally, fostering strong links whilst working across Royal Trinity Hospices community catchment area supporting patients with urgent and complex specialist palliative care needs wherever they call home.

Main duties of the job
  • Carry out holistic assessment of patients specialist palliative care needs and develop an initial care plan to address any urgent needs.
  • Communicate and liaise with the patients usual care team including their GP, District Nurses, existing specialist palliative care professionals, out of hours services, reporting on the condition and needs of the patients and providing expert advice and support to address their urgent needs.
  • Establish excellent working relationships with internal and external peers and colleagues to foster positive and collaborative working.
  • Offer support, guidance, and education to less experienced members of the rapid response team, and other professional visitors to the team to encourage individual and team learning and development in specialist palliative care.

  • The successful candidate must be able to travel with ease across our catchment area (a current UK driving licence is essential).
