Job description

Clinical Nurse Specialist for Stroke Services

We have an exciting opportunity has arisen to join the stroke specialist nursing at Hereford hospital.

The post holder will take a key role in the Stroke specialist nursing team. Using advanced nursing practice, manage a cohort of patients referred for specialist early intervention, care and support within Stroke services. The post holder will be a core member of the Stroke multi-disciplinary team (MDT) and will work across all elements of the Stroke/TIA pathway, which includes the Emergency Department through to either rehabilitation or palliative care.

Main duties of the job
  • Demonstrate advanced practice skills in the holistic assessment, planning, implementation and evaluation of care for the cohort of patients within the Stroke speciality.
  • Develop increased levels of autonomy and advocacy, using analytical and judgemental skills in undertaking differential diagnoses to ensure the delivery of appropriate care.
  • Transfer patients for necessary investigations.
  • Undertake clinical competencies relevant to the stroke service to demonstrate expertise in extending nursing roles.
  • Facilitate and coordinate the process of informed choice of patients in the provision of treatment and care.
  • Act as a resource for health care professionals working in the Trust providing specialist advice and support related to patients diagnosed with either a Stroke or TIA.
  • Manage a cohort of patients and prioritise workload to meet both the patients and service needs.
  • Refer to other health professionals and outside agencies to ensure optimum care/support to achieve a seamless transition from primary and secondary care.
