Job description

Clinical Nurse Specialist (CNS) / Clinical Paramedic Specialist (CPS)

Are you an experienced community palliative care nurse or paramedic looking for an exciting opportunity to join a new initiative within the Hospital@Home Team, hosted within the St Christophers Community Palliative Care Team?

Would you like to be part of a dynamic team that provides the best possible palliative care for people living in South East London? Our team responds rapidly, is holistic, reflexive, responsive, and utilizes all your clinical skills to provide high-quality palliative care through direct care or by supporting other clinicians via teaching, education, and advancing practice.

Main duties of the job

Join us in this innovative role where you'll support people to remain in their own homes rather than attending hospital, or facilitate rapid discharge when appropriate, using the Hospital@Home model. You will be an integral part of the Bromley Hospital@Home Service and the St Christophers Community Palliative Care Team, working closely with local acute and inpatient teams.

About us

St Christophers is an extraordinary place to work. We are committed to delivering high-quality care and support to our patients, their families, and carers. We believe that our staff can only provide this if they feel supported in their endeavors.

Job responsibilities

Position: Community Clinical Nurse Specialist (CNS) / Clinical Paramedic Specialist (CPS) (Hospital@Home)

Location: Bromley, with possible work from the Sydenham site

Hours: Part-time, 22.5 hours per week, fixed-term or secondment opportunity covering maternity leave

Salary: £32,250.50 - £36,390.50 (including HCA, for 22.5 hours/week). Salary will be determined based on experience and service.

We are looking for experienced community palliative care nurses or paramedics to join our team, supporting our innovative Hospital@Home service, which aims to prevent hospital admissions, enable early discharge, and support patients at home.

What will you be doing?

You will work across organizations to support individuals to stay at home or facilitate rapid discharge, supporting those reaching the end of life through the Palliative Pathway. You will work closely with the Bromley Hospital@Home Team, St Christophers Community Palliative Care Team, and other local health services.

Benefits of joining St Christophers
  • Stimulating and rewarding career with opportunities to make a real difference
  • Access to excellent training and development, including master's level education and non-medical prescribing
  • Health and wellbeing schemes
  • Contributory pension scheme and life assurance with generous benefits
