Job description

Job Title: Breast Care Nurse

An exciting opportunity has arisen for the role of a Breast Care Nurse in this dynamic and challenging specialty. You will play a key role in the delivery of the Breast Care Service, in our dynamic Nursing team, supporting Breast Cancer patients throughout their cancer journey. The successful candidate will work towards managing an independent caseload of patients, working collaboratively with a wide range of healthcare professionals.

Responsibilities:

  1. Provide a specialist and advanced nursing contribution to the clinical management of patients with breast cancer across all boundaries.
  2. Deliver a specialist Nurse-led service for patients with a Family History of Breast Cancer and Breast Pain.
  3. Supervise and lead the team with patient-centred follow-up management.
  4. Act as a resource for the multi-professional teams involved in the care and management of patients with a breast cancer diagnosis.
  5. Maintain and develop the Breast Care Nursing service at Walsall Healthcare Trust, establishing good working relationships with clinicians and other key staff.
  6. Be part of the multi-professional breast cancer tumour group within the Trust, Network, and Region (where required), developing standards of care for patients diagnosed with breast cancer.
  7. Provide effective and efficient service, delivering specialist advice and supportive care to patients and their families.
  8. Contribute to Breast care follow-up focusing on the physical, sexual, psychological, and social issues of care.
  9. Attend and be a core member of the Breast MDT meetings in accordance with national guidance.
  10. Manage own workload across a wide range of specialist activities including organizing case conferences and liaising with other healthcare professionals.
  11. Assist with the implementation of Treatment Summaries.
  12. Develop and implement personal treatment plans that empower patients in their follow-up care.
  13. Coordinate and manage patients undergoing treatment at Walsall Manor Hospital to support patients both on and off treatment.
  14. Provide information to patients related to treatment/follow-up and signpost to appropriate healthcare providers.
  15. Provide telephone advice and triage where appropriate, documenting and communicating outcomes as per service specifications.
  16. Provide level 2 psychological support within knowledge boundaries following appropriate training.
  17. Perform Holistic Needs Assessments using designated screening tools as appropriate.
  18. Collate patient experiences and develop action plans.
  19. Develop and facilitate Health and Well Being Programmes/Beyond Cancer, including patient and carer education.
  20. Document all interactions with accuracy in line with Trust policy and procedure.
  21. Manage existing follow-up clinics and create new clinics within the self-supported management structure and the Macmillan vision.
  22. Demonstrate excellent communication skills both verbal and written.

Professional Development:

This 12-month developmental post incorporates leadership and management training. Upon successful completion of training requirements and a competency framework, a Band 7 senior clinical nurse specialist post will be offered.
