A healthcare organization is looking for a Clinical Nurse Specialist to join their Specialist Team in London. You will provide expert nursing services and collaborate with multidisciplinary teams to enhance patient care quality. Job Summary: In line with the Trust's Annual Business Plan, you will work as part of a Specialist Team providing expert nursing/midwifery practice and cost-effective services to patients requiring routine and emergency care, from outpatients and pre-admission to discharge and review at home across primary and secondary care. The Division aims to foster an environment conducive to staff development and service quality improvements through high standards of training, teaching, and clinical care within the acute/community setting. Under the guidance of the Clinical Nurse Specialist, make clinical judgments involving assessment, planning, and treatment implementation, ensuring access to appropriate advice and support for specialist patients and their carers.
Expectations: Collaborate with the multidisciplinary team across an integrated care pathway to deliver quality standards aligned with NICE guidance. Ensure equitable access to investigations, treatment, and follow-up care for all patients. Develop best practices and guidelines in line with NICE guidance, and collect data to monitor compliance. Use audits to improve practice and evaluate services, and implement recommendations from national audits and benchmarking. Identify personal professional development objectives reflecting local and national service needs, seeking advice and support as appropriate.
Performance: Demonstrate specialist knowledge and skills, sharing expertise to positively influence patient care. Develop personal clinical knowledge and skills, and contribute to the development of other staff. Reflect on practice through supervision and mentorship. Provide comprehensive nursing care for patients within the specialty, working closely with the multidisciplinary team to meet patient and family needs for seamless care. Use clinical judgment for decision-making, including history taking and physical assessments. Assist in service development, promote patient participation in decision-making, and contribute to audits. Recognize deteriorating patients and take appropriate actions, liaising with the team as needed. Obtain valid consent prior to investigations and treatments.
Workforce: Attend relevant meetings, represent the team, and contribute to the education, training, and development of clinical staff. Help implement NICE guidance to ensure ongoing staff education and effective interprofessional relationships.
