An exciting opportunity has arisen for a motivated, dynamic and proactive nurse to join our ACHD team at Guy's and St Thomas' Hospital.
We are looking for a nurse with a strong background in caring for adult cardiac patients, ideally with experience in the care of adults with congenital heart disease (CHD) and an interest in transition and CHD interventions.
The successful candidate will provide expert clinical skills, knowledge, support and advice to patients with CHD and their relatives as they move through outpatient and inpatient services and across the network.
The role will include responsibilities for the ongoing clinical management of patients in nurse-led and multidisciplinary clinics, during in-patient episodes, and in the community through liaison with local health providers. You will also provide specialist support and advice to healthcare professionals within the Trust.
The successful candidate will be expected to work both autonomously and as part of a specialist multidisciplinary team. Excellent communication and organisational skills are essential. The role involves off-site travel to support outreach clinics.
Ongoing support will be provided by the ACHD consultants and nurse specialist team.
The candidate must demonstrate post-registration study and postgraduate level working via a continuous development portfolio.

Main duties of the job
The post holder will develop, implement, and evaluate a seamless specialist service, ensuring patients receive high-quality clinical care. They will act as a lead specialist, providing advice, education, and support to healthcare professionals and patients. The post holder will undertake clinical audits and research within the specialist area.
The post holder will contribute to the development of clinical practice, research, and standards of care, including policy, protocols, and guidelines, in collaboration with multidisciplinary colleagues.
They will also ensure the safety and well-being of service users, promoting dignity and human rights through person-centered care principles.
