Job overview

An exciting opportunity has arisen for a motivated, dynamic and proactive nurse to join our ACHD team at Guy's and St Thomas' Hospital.

We are looking for a nurse with a strong background in caring for adult cardiac patients, ideally with experience in the care of adults with congenital heart disease (CHD) and an interest in transition and CHD interventions.

The successful candidate will provide expert clinical skills, knowledge, support and advice to patients with CHD and their relatives as they move through outpatient and inpatient services and across the network.

The role will include responsibilities for the ongoing clinical management of patients in nurse-led and multidisciplinary clinics, during in-patient episodes, and in the community through liaison with local health providers. You will also provide specialist support and advice to healthcare professionals within the Trust.

The successful candidate will be expected to work both autonomously and as part of a specialist multidisciplinary team. Excellent communication and organisational skills are essential. The role involves off-site travel to support outreach clinics.

Ongoing support will be provided by the ACHD consultants and nurse specialist team.

The candidate must demonstrate post-registration study and postgraduate level working via a continuous development portfolio.

Main duties of the job

The post holder will develop, implement, and evaluate a seamless specialist service, ensuring patients receive high-quality clinical care. They will act as a lead specialist, providing advice, education, and support to healthcare professionals and patients. The post holder will undertake clinical audits and research within the specialist area.

The post holder will contribute to the development of clinical practice, research, and standards of care, including policy, protocols, and guidelines, in collaboration with multidisciplinary colleagues.

They will also ensure the safety and well-being of service users, promoting dignity and human rights through person-centered care principles.

Detailed job description and main responsibilities

The role includes:

• Providing clinical support and advice lines to adult congenital heart disease patients and carers (telephone/email).
• Supporting inpatient review and management, contributing to care plans, and communicating with patients and relatives.
• Serving as a resource for healthcare professionals within the Trust and across the network.
• Running face-to-face and virtual clinics for specific patient groups (transition nurse-led clinics).
• Pre-assessing and educating patients undergoing interventions or surgery.
• Collaborating with anesthetics regarding patient safety and care planning for complex cases requiring cardiac and non-cardiac surgery.
• Attending and contributing to multidisciplinary team (MDT) meetings.
• Providing specialist education and training to nurses, students, midwives, and allied health professionals to support care for patients with congenital heart disease.

Person specification

Knowledge/Qualification

• RN1 with BSc first degree
• Professional knowledge at degree level, supplemented by specialist training, experience, and short courses up to Masters level equivalent
• Post-registration qualification in a relevant specialty or equivalent experience
• Leadership course
• Non-medical physical assessment course
• Working towards a master's degree

Experience

• Extensive autonomous working experience at an advanced level within the specialist area, delivering effective patient-centered care.
• Experience in audit, research, and evidence-based practice.
• Proven management/leadership skills and autonomous practice.
• Experience supervising students and providing clinical education at undergraduate level.
• Experience in patient advocacy and change management.

Skills

• Advanced clinical skills in the specialty
• Excellent listening and counseling skills, capable of managing complex and sensitive situations.
• Teaching skills, especially in patient education about managing their conditions.
• Up-to-date knowledge of clinical and professional issues related to adult congenital heart disease (NHS England).
