Job responsibilities

We are seeking a Clinical Nurse Specialist to join our Palliative Hub Team in a part-time role, working 24 hours per week. The work can primarily be performed from home, with flexibility regarding the contract location being either Cambridge or Wisbech.

The Palliative Hub service provides specialist palliative and end-of-life care to adults (18+) registered with a GP within the Cambridge and Peterborough Integrated Care Board, via a telephone advice line. Our goal is to help patients remain in their usual residence for as long as appropriate and safe, supporting patients, carers, and healthcare professionals to avoid unnecessary hospital admissions.

This service encourages calls from out-of-hours services, on-call doctors (HUC), the East of England Ambulance Service, and other community services.

Please note, this is not a patient-facing role.

The successful candidate will holistically assess, plan, implement, and review therapeutic interventions for patients contacting the advice line, with the aim of avoiding unnecessary hospital admissions.

Willingness to undertake prescribing qualifications is essential unless already qualified as a non-medical prescriber.

The role involves working across a 7-day period, providing 24-hour advice and support as part of a rota, with at least monthly visits to the hospice.

Starting salary of £47,071, with opportunities to progress to £53,866.

Please note that the salary quoted is based on full-time hours and will be pro-rata for part-time posts. New starters usually commence at the minimum of the salary band, but exceptional candidates with significant relevant strengths may negotiate higher.

Person Specification

Knowledge & Qualifications

  • Registered General Nurse, First Level degree
  • Prescribing qualification or willingness to undertake prescribing training

Experience

  • Holistic assessment experience
  • Advanced care planning experience
  • Experience working within a multidisciplinary team
