Overview
We are looking for a dynamic, enthusiastic, resilient and dedicated Clinical Lead Nurse Manager to join our Integrated nursing and therapy Urgent Community Response team in North Norfolk, to support daily clinical and operational staff management. This is a fast-paced but rewarding role that would suit an experienced nurse with a background in acute, community or significant experience in other urgent and emergency response teams. The role provides opportunities for new challenges and ongoing development in a multi-disciplinary, innovative, supportive and friendly team who work closely with other community and NHS services to prevent admission to hospital.
The successful candidate will have excellent communication skills and work collaboratively to meet the needs of the service. You must demonstrate evidence of professional competencies and be able to apply your knowledge and skills to clinical practice and share your knowledge with the team in a range of ways.
Job Purpose
- To provide clinical expertise, clinical leadership and line management to staff working within Urgent Community Response (UCR).
- To support staff to achieve and ensure competencies are maintained within the services.
- To contribute to delivering high standards of health care to patients within their own places of residence.
- To ensure that Trust annual priorities and quality goals are embedded within the workforce by ensuring that all annual appraisals are complete within the team.
Hours of Operation
The service operates between 08:00 and 20:00 seven days a week and you will be required to work shifts between these times.
You will positively contribute to service and quality development through new ways of working. The post holder will work alongside the Clinical Operations Manager and UCR Nursing and Therapy Leads to support operational, clinical and professional development of staff and the service, who work across the North Norfolk area.
The UCR teams provide admission avoidance services via the Rapid Response MDP (formerly NEAT) and 2-hour UCR response process. We also support patients at the end of life and via the discharge to assess pathway.
Our service is participating in current and future innovative projects to support the ongoing need to keep patients out of hospital and care for them at home when possible, and so you will be expected to contribute to supporting Virtual Ward patients and the recently formed award-winning Unscheduled Care Coordination Hub.

Main Duties and Responsibilities
- Provide professional and clinical leadership with a supportive and flexible leadership approach to promote confidence in the team and Service.
- Clinically supervise and line manage the UCR Team ensuring staff adhere to the Trust values and the NMC code.
- Work with and deputise for the Service Lead as required.
- Engage and lead caseload management by overseeing, monitoring and evaluating care plans for all patients to ensure quality care, treatment progression and timely discharge.
- Provide overview of remote monitoring systems within UCR and liaise with UCCH and VW Hub to support service delivery.
- Support the Clinician of the Day (COD) with delegation of tasks and prioritization of work based on clinical acuity.
- Work within the integrated team to prevent unnecessary admission to hospital and facilitate early discharge from hospital.
- Collaborate with all health care professionals and statutory/non-statutory agencies to provide a seamless, integrated service to service users.
- Support community clinicians and students through supervision, appraisal and mentorship in line with Trust policy.
- Oversee that clinical supervision is occurring within the team and accurately recorded.
- Liaise with the UCR Clinical Operations Manager (COM) to identify gaps in management and offer support as needed.
As part of operational leadership for the locality you will be required to:
- Oversee the allocation of work for clinicians.
- Ensure all staff are competent with mobile working.
- Engage and lead on triage and referral to discharge standard processes.
- Work to a demand and capacity model which reflects workload needs at any given time.
Clinical
The post holder will oversee the following Core Clinical Interventions:
- Assessment, planning, implementation and evaluation of all aspects of patient care, and development of treatment/care plans that may need to be delivered from a range of options.
- Develop new skills in care delivery, using innovative techniques and sharing these with the wider teams ensuring competencies are met.
- Following a holistic assessment, devise an individualised care programme for each patient, modifying it as required.
- Assess patients\' holistic needs, communicating highly complex and sensitive information to patients and carers regarding their assessment, diagnosis, prognosis and treatment plan.
- Become specialist in Assistive Technology and utilise it in patient care where appropriate.
- Adjust explanations to overcome barriers to understanding (hearing impairment, mental capacity impairment, etc.) to gain understanding, consent and concordance.
- Use tact and persuasive skills to gain patient cooperation in treatment/care/management plan.
- Liaise with UCR, UCCH & Virtual Ward clinicians, GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care coordinated appropriately.
- Provide patient-focused education, training and advice to patients, carers, other professionals and voluntary organisations.
- Participate in multi-disciplinary/multi-agency meetings as appropriate (e.g., Gold Standard Framework).
- Plan and organise specialist services within the team.
- Provide clinical expertise and advice to others, including Social Services, GPs and other agencies.
- Ensure activity data and clinical information is recorded on SystmOne and completed daily.
- Adhere to universal precautions and infection control policies where bodily fluids or infectious material are involved.
- Ensure informed consent is obtained prior to initiating interventions.
- Work with staff to ensure competencies are maintained.
- Maintain clinical credibility by acting regularly as a clinical practitioner in own area of expertise.
Professional Leadership
- Undertake risk assessments and act upon them appropriately.
- Ensure workload for self and others is planned and prioritised according to service targets and needs.
- Maintain and manage paperwork and records for self and team.
- Monitor equipment and supplies via electronic ordering systems.
- Work with and deputise for the Service Lead.
- Engage and lead on triage and referral to discharge standard processes.
- Participate in integrated team meetings; chair meetings as necessary.
- Develop and maintain effective working relationships with all colleagues and external partners.
- Contribute to operational strategy and project management with Clinical Operational Managers.
- Participate in local and organisation-wide working groups.

- Mentor staff and provide training in specialist areas; demonstrate leadership skills.
- Relay information from management to team on a regular basis and assist in resolving conflicts and investigations as required.

- Contribute to modernisation and forward movement of NCH&C, understanding local and national policy implications.
- Demonstrate sound clinical governance and risk management; document risk associated with change.
- Manage absence per Trust policy and ensure mandatory training and appraisals for the team, achieving KPIs.
- Act as an authorised signatory for goods, services and payroll.
- Complete HR documentation for team members within designated timeframes; report data returns and analyze activity and quality data.
- Monitor delegated team budget and assist Locality Management Team with resource management.
- Manage staff absence, mandatory training, and leave; inform Locality Manager of significant trends.
- Comply with organisational policies and participate in updating them as appropriate.
- Ensure policies are interpreted correctly and implemented by designated staff.
