Job description

Active Care Group’s Care in the Home division continues to grow, providing safe, reliable, and outcome-based care delivered by our exceptional support workers, clinicians, and management team.

Calling all clinicians: Are you tired of working 12-hour days/nights and seeking a better work-life balance?

Due to ongoing growth, we are seeking an additional Clinical Manager to join our Care in the Home division, where we proudly employ Nurses as Clinical Managers.

This is an exciting opportunity as we have ambitious plans for expansion in our division, which supports children, young people, and adults with conditions such as brain injury, spinal cord injury, neurological and neurodegenerative diseases, respiratory, and gastrointestinal conditions, including PEG feeding.

We seek someone with strong clinical skills, leadership qualities, and a passion for teaching and mentoring, to work closely with a team of professionals ensuring individuals with complex needs can live safely and comfortably at home.

Please note: We cannot sponsor candidates requiring a UK work visa for this role.

What you'll be working:
  • Monday - Friday, with on-call duties for emergencies
  • Home-based role with regional travel (Chester, Llanfairfechan, Cyfflliog, Ruthin, Saughall, Flint, Liverpool, Preston)
What you'll be doing:
  • Providing competency-based training to support clients with complex needs, promoting independence at home.
  • Delegating clinical skills to Support Workers and Care Managers through training and assessment.
  • Updating and maintaining Personal Care Plans in line with company protocols.
  • Conducting annual observations and ensuring all staff competencies are current.
  • Handling safeguarding issues calmly and accurately.
  • Communicating regularly with ICBs and care funders.
  • Monitoring care risks and conducting audits of records and MAR charts.
  • Visiting clients at home to teach competencies and address issues.
  • Supporting staff welfare and managing workload concerns.
  • Thinking laterally and understanding the broader scope of complex care.
What you'll have:
  • Current NMC registration (Dual Qualification RN/RSCN) or Adult Nurse with recent experience with children.
  • Legal right to work in the UK.
  • Full driving license and own vehicle.
  • Knowledge of vents, bowel and bladder management, tracheostomy, catheterization, epilepsy, blood glucose monitoring, NIVs, Nebs, PEG, challenging behaviour, ADHD, and autism.
