Active Care Group's Care in the Home division continues to grow, providing safe, reliable, and outcome-based care to clients through our exceptional support workers, clinicians, and management team.

Main duties of the job

Calling all clinicians! Are you tired of working 12-hour shifts and seeking better work-life balance? Due to our continued growth, we are seeking an additional Clinical Manager for our Care in the Home division, where we proudly employ nurses as Clinical Managers. This is a great opportunity to join us as we expand our services supporting children, young people, and adults with conditions such as brain injury, spinal cord injury, neurological and neurodegenerative diseases, respiratory issues, and gastrointestinal conditions, including PEG feeding. We are looking for someone with strong clinical skills, leadership qualities, and a passion for teaching and mentoring, working closely with a team of professionals to ensure individuals with complex needs can live safely and comfortably at home. Please note, we cannot offer sponsorship for this role, and candidates will need the right to work in the UK.

Job responsibilities

What you'll be working:

  • Monday - Friday (including being part of the on-call rota for emergencies)
  • Home-based with travel across a designated region (Chester, Llanfairfechan, Cyffylliog, Ruthin, Saughall, Flint, Liverpool, Preston)

What you'll be doing:

  • Providing competency-based training to support clients with complex needs, enhancing their ability to live independently at home.
  • Delegating clinical skills to Support Workers and Care Managers through training and assessment, ensuring safe practice.
  • Collaborating with the Care Manager team to keep Personal Care Plans up-to-date, accurate, and compliant.
  • Documenting delegation of clinical skills and conducting annual observations of Support Workers and Care Managers.
  • Ensuring all support workers have current competencies and handling safeguarding issues calmly and accurately.
  • Communicating with ICBs and care funders, and liaising with scheduling and care teams about risks.
  • Auditing records and MAR charts monthly within your geographical area.
  • Visiting clients to teach competencies, ensure safety, and resolve issues.
  • Maintaining accurate documentation and supporting support workers’ welfare, especially lone workers.
  • Demonstrating leadership, lateral thinking, and a broad understanding of complex care.

What you'll have:

  • A current NMC Pin registration (Dual Qualification RN/RSCN) or Adult Nurse with recent experience working with children.
  • Legal right to work in the UK.
  • Full driving license and own vehicle.
  • Knowledge of vents, bowel and bladder management, tracheostomy, catheterization, epilepsy, blood glucose monitoring, NIVs, Nebs, PEG, challenging behavior, ADHD, and autism.
