Position Summary

Baptist Health Brookwood Hospital

The Clinical Liaison at Baptist Health Brookwood Hospital acts as a bridge between internal and external referral sources and the inpatient rehabilitation facility. This role involves establishing relationships with stakeholders such as physicians, payers, and referral sources to ensure appropriate patient placement. The Clinical Liaison conducts clinical assessments and screens potential candidates in collaboration with the Medical Director or rehabilitation physician.

Responsibilities

Essential Functions

  1. Assist and coordinate the referral and pre-admission process, including documenting patient assessments regarding reasons for admission in compliance with CMS regulations. Identify potential patients early through effective communication with hospital care teams, physicians, and other referring facilities, including clinical rounds.
  2. Conduct thorough on-site clinical evaluations of patients, including chart reviews, physical screenings, and interactions with involved healthcare professionals and family members to gather comprehensive information for admission decisions.
  3. Demonstrate knowledge of growth, development, and aging processes to provide age-appropriate care planning.
  4. Gather information about patients’ resources and benefits, and obtain insurance authorizations for admission, including negotiations for workers’ compensation when applicable.
  5. Communicate facility and program information to patients and families, including expectations and participation requirements, and arrange facility tours as needed.
  6. Notify referral sources about admission decisions and transfer schedules promptly.
  7. Coordinate transitions from referring facilities or home to the rehabilitation facility.
  8. Establish and maintain contacts with physicians, care teams, and other healthcare professionals to facilitate timely admissions.
  9. Complete educational in-services with physicians, case managers, and other professionals.
  10. Contribute to market analysis, identify opportunities, and support marketing plans, monitoring their success.
  11. Maintain reports on referrals, admissions, denials, and census data.
  12. Maintain consistent, punctual attendance in accordance with organizational policies and relevant laws.
  13. Ensure compliance with all organizational policies and procedures.
Qualifications

Education/Training: Degree from an accredited school and licensure in a healthcare-related field.

Licensure/Certification: Current license in Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Respiratory Therapy, or Licensed Clinical Social Work in Alabama. Valid Alabama driver’s license required.

Experience: Minimum of two years of clinical experience with skills in assessment and marketing. Preferably, two years in acute care or rehab settings. Knowledge of physician relations, insurance interfacing, and local medical communities. Strong communication skills, ability to work harmoniously with others, and following written and oral instructions are essential.
